Your parent has three specialist appointments this month, a weekly blood draw, and a physical therapy session every Tuesday. They stopped driving six months ago. You have already burned through your PTO covering transport runs. Getting a senior to medical appointments is one of the most persistent logistical problems in eldercare — and California has more transportation options than most families realize.
Medi-Cal Non-Emergency Medical Transportation (NEMT)
If your parent has full-scope Medi-Cal, non-emergency medical transportation is a covered benefit. NEMT covers rides to and from Medi-Cal-covered medical appointments, including doctor visits, dialysis, physical therapy, and mental health services.
How to arrange NEMT:
- Call your parent's Medi-Cal managed care plan's member services line
- Request NEMT for the specific appointment date, time, and location
- Provide information about your parent's mobility needs (wheelchair, stretcher, ambulatory)
- Schedule the ride at least 48-72 hours in advance (urgent requests are handled case-by-case)
The managed care plan contracts with transportation brokers who dispatch appropriate vehicles — sedans for ambulatory patients, wheelchair-accessible vans, or stretcher transport for non-ambulatory patients. Round-trip service is standard.
Important distinction: NEMT covers medical appointments only. Grocery runs, social visits, and errands are not covered under this benefit. For those needs, other programs apply.
Paratransit (ADA-Mandated)
Under the Americans with Disabilities Act, every public transit agency in California must operate a complementary paratransit service for individuals who cannot use fixed-route buses or trains due to a disability. This includes many aging seniors with mobility, vision, or cognitive impairments.
Eligibility: Your parent must apply through the local transit agency. The application typically requires a medical professional's certification of the disability that prevents fixed-route transit use. Approval grants door-to-door service within the transit agency's service area.
Cost: Paratransit fares are typically double the regular fixed-route fare — usually $2 to $6 per one-way trip. For seniors who qualify, this is far less expensive than private transport.
Limitations: Rides must be scheduled one to three days in advance. Pick-up windows are typically 20-30 minutes. Service areas are limited to three-quarters of a mile from existing bus routes.
Area Agency on Aging Transportation Programs
Local Area Agencies on Aging (AAAs) coordinate transportation programs for seniors 60 and older. Services vary by county but commonly include:
- Volunteer driver programs where trained volunteers provide door-to-door transport to medical appointments, grocery shopping, and community activities
- Senior shuttle services operating fixed routes to medical facilities, senior centers, and shopping areas
- Transportation vouchers that subsidize taxi or rideshare costs
