Tennessee Rural Medicare Savings Assistance: How to Apply Without a Nearby Office
In rural East and West Tennessee, the nearest county DHS office can be a 45-minute drive. For a senior on a fixed income without reliable transportation, that drive is not a minor inconvenience — it is a barrier that prevents them from applying for programs that would save them $2,434.80 a year.
Tennessee's Medicare Savings Programs do not require an in-person visit. Every step — application, document submission, renewals — can be done remotely.
Phone Application: Often the Fastest Route
Calling TennCare Connect at 1-855-259-0701 (TTY: 1-800-848-0298) puts a customer service representative on the line who walks through the entire application verbally. No computer needed, no driving.
The representative enters the information into TennCare's system in real time. At the end of the call, the application is submitted. If electronic data matching verifies income and assets, no follow-up documents are needed.
For rural seniors who are comfortable on the phone but not on a computer, this is consistently the smoothest path.
Online Application Through TennCare Connect
The TennCare Connect portal at tenncareconnect.tn.gov works from any device with internet access. An adult child in Nashville or Atlanta can complete the application for a parent in Hardin County or Hancock County without either person leaving home.
The portal requires multi-factor authentication and can be frustrating for first-time users. If an out-of-state adult child is doing this on behalf of a parent, they will need to complete the Authorized Representative section and file Form HCFA to gain access to the parent's case information.
Documents can be uploaded directly through the portal — photos of bank statements, Social Security letters, and Medicare cards taken on a phone work fine.
Paper Application by Mail or Fax
The paper application — "Application for Health Coverage & Help Paying Costs" — can be mailed to TennCare Connect, P.O. Box 305240, Nashville, TN 37230-5240, or faxed to 1-855-315-0669.
Paper applications take longer to process because TennCare must manually enter the data. But for someone without reliable phone service or internet, mail remains an option.

Regional AAADs: In-Person Help Closer to Home
Tennessee's nine Area Agencies on Aging and Disability (AAADs) operate closer to rural communities than some county DHS offices. AAAD staff provide non-adjudicative counseling — they do not approve applications, but they help fill out forms, gather documents, and navigate the system.
The statewide AAAD locator is at tn.gov/disability-and-aging/resource-maps/aaad-locator.html or can be reached by calling 1-866-836-6678. SHIP counselors, who provide free Medicare-specific guidance, operate through the same AAAD network and can be reached at 1-877-801-0044.
For rural seniors in Appalachian communities where applying for "TennCare" carries social stigma, framing the conversation around Medicare premium help rather than Medicaid can reduce resistance. Medicare Savings Programs pay Medicare costs — and MSP benefits alone do not trigger estate recovery against the family home.
What Rural Seniors Need to Have Ready
Regardless of application method, having these items on hand speeds processing:
- Social Security Award Letter (shows gross monthly benefit)
- Medicare card (red/white/blue card with the MBI number)
- Bank statements from the past 30–60 days for all accounts
- Pension or VA benefit statements if applicable
- Proof of Tennessee residency (utility bill, lease, or driver's license)
TennCare's electronic data matching often makes paper documents unnecessary, but having them ready prevents delays if the system flags a discrepancy.
The Tennessee Medicare Savings Programs & Extra Help Guide provides a complete document checklist and step-by-step instructions for each application method — designed so an adult child in another state can walk a parent through the process by phone.
