Tennessee Extra Help: Part D Low-Income Subsidy Eligibility and Application
A senior filling a maintenance prescription at a Tennessee pharmacy gets hit with a $85 copay they weren't expecting. Their Part D plan has a deductible phase, a coverage gap, and cost-sharing tiers that make out-of-pocket costs unpredictable until they hit the catastrophic threshold. Extra Help — formally the Part D Low-Income Subsidy — was designed to eliminate most of that uncertainty, but the majority of eligible Tennesseans don't know it exists or assume they earn too much to qualify.
What Extra Help Covers in 2026
Full Extra Help eliminates or sharply reduces every layer of Part D cost-sharing:
- Part D premium: covered up to the regional benchmark amount (if your parent's plan premium exceeds the benchmark, they pay the difference)
- Part D deductible: eliminated entirely (standard deductible is $615 in 2026)
- Copays: no more than $12.65 for covered brand-name drugs, with reduced copays for generics
- Late enrollment penalties: waived completely
Under the Inflation Reduction Act, all qualifying LIS applicants now receive full Extra Help benefits — the old partial-subsidy tier has been eliminated.
Two Paths to Extra Help
Automatic eligibility through a Medicare Savings Program. Anyone enrolled in QMB, SLMB, or QI through TennCare is automatically deemed eligible for Extra Help. No separate application required. The same TennCare approval that stops the Part B premium deduction also triggers Extra Help enrollment through CMS.
Direct application through Social Security. Seniors whose income exceeds MSP thresholds but falls below 150% of the Federal Poverty Level can apply for Extra Help on its own. The 2026 limits are higher than the MSP limits:
| Individual | Couple | |
|---|---|---|
| Monthly income limit | Below $1,995 | Below $2,722.50 |
| Resource limit | $16,590 | $33,100 |
The resource limits for standalone Extra Help are significantly more generous than the MSP limits ($9,950/$14,910). This means a senior with $12,000 in savings who doesn't qualify for QMB because of income could still qualify for Extra Help directly.
Apply online at ssa.gov, by phone at 1-800-772-1213, or by submitting Form SSA-1020.
The MSP-to-Extra Help Connection
Filing an Extra Help application through Social Security can also trigger a Medicare Savings Program screening at the state level. Under Section 113 of MIPPA, when someone applies for Extra Help and consents to MSP screening, SSA transmits their data to TennCare. The Extra Help filing date becomes the official MSP filing date.
However, this data transfer can take months. If your parent might qualify for both programs, applying directly to TennCare Connect alongside the Social Security Extra Help application is faster and avoids the transmission delay.

When Extra Help Kicks In at the Pharmacy
After approval — whether through automatic MSP deemed status or a direct Social Security application — CMS updates the Medicare Part D plan's records. The plan applies the subsidy once its records show the approval; if there is a transmission delay, use the Best Available Evidence (BAE) policy.
If your parent fills a prescription during the gap between approval and system update, the Best Available Evidence (BAE) policy applies. By presenting one of these at the pharmacy, the Part D plan is legally required to adjust cost-sharing on the spot:
- TennCare Notice of Eligibility showing QMB, SLMB, or QI approval
- A printout from TennCare Connect showing active enrollment
- A state buy-in transaction confirmation record
The pharmacist enters the proof, the plan adjusts the copay, and the difference is applied immediately — no need to pay full price and wait for a reimbursement.
Sorting Out MSP vs. Extra Help
The two programs overlap but serve different functions:
- Medicare Savings Programs (QMB, SLMB, QI) pay the Part B premium and, in QMB's case, all Medicare cost-sharing. They're state programs run by TennCare.
- Extra Help reduces Part D prescription drug costs. It's a federal program administered by Social Security and CMS.
MSP enrollment automatically qualifies someone for Extra Help, but Extra Help enrollment does not automatically qualify someone for an MSP. The income and asset thresholds are different, and the application routes are different.
The Tennessee Medicare Savings Programs & Extra Help Guide covers both programs together — the eligibility math, application steps, and what changes at the pharmacy and in your parent's Social Security check once they're enrolled.
