Tennessee DSNP and Dual Eligible Benefits: What Medicare Savings Program Enrollees Should Know

If your parent is enrolled in a Medicare Savings Program in Tennessee, insurance agents may start calling about D-SNP plans. These are Dual Eligible Special Needs Plans — a type of Medicare Advantage plan designed specifically for people who have both Medicare and some form of Medicaid, including QMB, SLMB, or QI.

Understanding what D-SNPs are, what they add, and what they replace helps families make an informed choice instead of reacting to a sales call.

What a D-SNP Is

A D-SNP is a Medicare Advantage plan (Part C) that replaces Original Medicare. Instead of using Parts A and B directly, your parent's care runs through the D-SNP's provider network. The plan may add benefits that Original Medicare lacks — dental, vision, hearing, transportation, over-the-counter allowances.

In Tennessee, D-SNPs are offered by managed care organizations like UnitedHealthcare, Wellpoint, and others. They are private insurance plans, not government programs. The agents selling them earn commissions.

How D-SNPs Connect to Medicare Savings Programs

D-SNPs serve beneficiaries who are eligible for both Medicare and Medicaid. MSP status can establish Medicaid assistance, but it does not by itself confirm eligibility for a particular plan; check the plan's requirements with SHIP.

A D-SNP does not replace the Medicare Savings Program. The MSP continues paying the Part B premium through TennCare's state buy-in. If your parent has QMB, the balance billing protections remain in effect. Extra Help continues covering Part D prescription costs.

What changes is how medical care is delivered. Instead of seeing any Medicare-accepting provider under Original Medicare, care goes through the D-SNP's network. Referrals may be required for specialists. The plan's formulary governs which drugs are covered and at what tier.

What D-SNPs Typically Offer Beyond Original Medicare

Depending on the plan, D-SNP extras may include:

  • Routine dental cleanings and basic procedures
  • Annual vision exams and eyewear allowances
  • Hearing exams and hearing aid coverage
  • Non-emergency medical transportation
  • Monthly over-the-counter product allowances
  • Meal delivery after hospital discharge
  • Telehealth services

These extras are genuine benefits. For a senior who needs dental work or hearing aids — things Original Medicare does not cover — a D-SNP can fill real gaps.

What to Watch For

Network restrictions. If your parent has established relationships with specific doctors, confirm those providers are in the D-SNP's network before enrolling. Switching plans mid-year is limited to specific enrollment periods.

Prescription formulary. The D-SNP's drug list may differ from a standalone Part D plan. A medication covered under the current plan might be on a higher tier or require prior authorization under the D-SNP.

The sales process. D-SNP agents are compensated per enrollment. Their pitch focuses on the extras (dental, vision, OTC allowance), which are real. But they have no role in the MSP application itself and cannot help your parent apply for QMB, navigate TennCare Connect, or appeal a denial.

Should Your Parent Enroll in a D-SNP?

That depends on their medical situation, provider relationships, and what gaps they need filled. The unbiased place to evaluate this is a free appointment with a Tennessee SHIP counselor at 1-877-801-0044. SHIP counselors do not sell insurance, earn no commissions, and can compare D-SNP options against staying on Original Medicare with a Medigap supplement.

The first priority is making sure your parent has the MSP itself — the Part B premium coverage and Extra Help that form the financial foundation. The Tennessee Medicare Savings Programs & Extra Help Guide covers that process end to end: eligibility, application, and what to expect after approval.
