State Plan Personal Care Oklahoma: Home Care Without a Waitlist
Your parent needs help with bathing, dressing, and cooking — but they don't need nursing-level care, and they can't afford to pay a home care aide out of pocket. Oklahoma's State Plan Personal Care (SPPC) program might be the answer most families don't know about.
Unlike the ADvantage Waiver, which has capped enrollment slots and potential waitlists, SPPC is a Medicaid state plan entitlement. That means if your parent qualifies, they get services. No waitlist. No slot caps. No competing with thousands of other applicants for a limited number of openings.
2026 SPPC Eligibility Rules
Income limit: $1,350 per month gross income for a single applicant (100% of the Federal Poverty Level, effective April 1, 2026). For married couples where both apply, the combined limit is $1,824/month.
Asset limit: $9,950 for a single applicant; $14,910 for married couples. This is significantly higher than the ADvantage Waiver's $2,000 limit.
Clinical requirement: Your parent must have a functional need for personal care assistance with Activities of Daily Living (ADLs) or Instrumental Activities of Daily Living (IADLs). They do not need to meet the Nursing Facility Level of Care (NFLOC) standard required by the ADvantage Waiver — the clinical bar is lower.
No lookback period: Unlike the ADvantage Waiver's 60-month Medicaid asset transfer lookback, SPPC has no lookback rule. This means prior asset transfers don't create penalty periods.
No home equity cap: SPPC does not impose a home equity interest limit. The family home is exempt regardless of its value.
What SPPC Covers
SPPC provides personal care assistants who help with:
- Bathing, grooming, and dressing
- Toileting and continence care
- Transferring (getting in and out of bed or chairs)
- Meal preparation
- Light housekeeping related to the care recipient's needs
- Medication reminders
SPPC does not cover skilled nursing, medical equipment, home modifications, or case management. For those services, the ADvantage Waiver is the pathway.
Who Can Be a Paid SPPC Caregiver?
Non-spouse, non-guardian family members can be hired as the paid personal care assistant. Adult children, siblings, and other relatives are eligible. This is similar to the ADvantage Waiver's CD-PASS option but without the spouse/guardian exception — spouses and guardians cannot be paid SPPC caregivers.

When SPPC Is the Better Choice
SPPC makes sense when:
- Your parent's income is below $1,350/month (most people living solely on SSI or minimum Social Security)
- They need help with daily tasks but not medical-level care
- You want services immediately without risking a waitlist
- They've made asset transfers in the past 5 years that would create lookback penalties under the ADvantage Waiver
SPPC also works as a bridge — if your parent's condition worsens and they eventually need nursing-level care, you can apply for the ADvantage Waiver while SPPC services continue.
How to Apply
Apply through the mySoonerCare online portal or contact the OKDHS Medicaid Services Unit CareLine at 1-800-435-4711. You'll need income verification, asset documentation, and identification. Once the financial application is processed, an OKDHS staff member evaluates your parent's functional needs to confirm eligibility.
Processing times are generally faster than the ADvantage Waiver because there's only one eligibility track (no separate clinical NFLOC determination) and no slot capacity constraints.
