South Dakota Medicare Costs 2026: Part B Premium, Deductibles, and How to Get Help Paying
The standard Medicare Part B premium for 2026 is $202.90 per month — $2,434.80 per year. For people who receive Social Security, the premium is generally deducted from the benefit before payment. For South Dakota seniors living on fixed incomes, that deduction can represent 15% or more of their total monthly income. And the Part B premium is just one piece of the cost picture.
2026 Medicare Cost Breakdown
Here's what Medicare costs in 2026 before any state assistance:
| Cost | 2026 Amount |
|---|---|
| Part B monthly premium | $202.90 |
| Part B annual deductible | $283 |
| Part A hospital deductible (per benefit period) | $1,736 |
| Part A monthly premium (if not premium-free) | $311 (30–39 quarters) or $565 (under 30 quarters) |
| Part D out-of-pocket cap | $2,100 |
Most people enrolled in Part B pay the standard premium unless a program such as an MSP pays it. The Part A hospital deductible applies to Medicare-covered inpatient care. If you lack 40 quarters of work history for premium-free Part A, the monthly Part A premium adds $311–$565, depending on your covered work quarters.
A single hospitalization plus a full year of Part B premiums can cost over $4,000 — a devastating amount on a $1,200 or $1,400 monthly Social Security check.
South Dakota Programs That Pay These Costs
South Dakota administers the federal Medicare Savings Programs through the Department of Social Services. Once coverage is in place, these programs pay covered Medicare costs directly; premiums deducted during processing may be refunded for eligible months.
QMB (Qualified Medicare Beneficiary) covers the Part B premium, Part B and Part A deductibles, and Medicare copays and coinsurance for covered services. It also pays the Part A premium if you don't have premium-free Part A. Countable income limit for 2026: $1,350/month single, $1,824/month couple.
SLMB (Specified Low-Income Medicare Beneficiary) covers the Part B premium only. Countable income limit: $1,351–$1,616/month single, $1,825–$2,184/month couple.
QI (Qualifying Individual) also covers the Part B premium. Countable income limit: $1,617–$1,816/month single, $2,185–$2,455/month couple. QI is funded by a capped federal block grant and approved on a first-come, first-served basis.
All three programs have a resource limit of $9,950 for individuals and $14,910 for couples in 2026. Your home, one vehicle, and irrevocable burial funds are excluded from the resource count.
The Income Math Most People Get Wrong
South Dakota DSS uses your gross Social Security amount — not the net deposit you see in your bank account. If your check shows $1,147.10 after the Part B deduction, DSS adds the $202.90 back to calculate $1,350 gross income. Then DSS subtracts the $20 general income disregard, bringing countable income to $1,330 — within the QMB threshold.
Many families assume they earn too much based on the posted income limits without accounting for this $20 disregard. That single calculation error can mean missing out on over $2,400 per year in premium savings.

The Extra Help Bonus
Enrolling in any South Dakota MSP automatically triggers full Extra Help for Part D prescription drug costs. Your Part D deductible drops to $0. For 2026, MSP-only beneficiaries pay no more than $5.10 for a generic or preferred multi-source drug and $12.65 for other covered drugs before reaching the $2,100 annual out-of-pocket threshold; people with full Medicaid may pay less.
How to Apply
File Form DSS-EA-270 at any South Dakota DSS office, online at eaportal.sd.gov, or by phone at 1-800-305-3064. DSS must issue a decision within 45 calendar days of submission. If approved for QMB, SLMB, or QI, SSA stops deducting the Part B premium from your Social Security check within 60–90 days and reimburses any premiums deducted after your eligibility date.
The South Dakota Medicare Savings guide includes the eligibility calculation worksheet, document checklist, and a premium refund tracker for monitoring the SSA reimbursement.
