$0 South Dakota — Medicare Savings Program Eligibility Checklist

South Dakota DSS Office Locations for Medicare Savings Applications

Applying for South Dakota's Medicare Savings Programs doesn't require a trip to Pierre. The Department of Social Services operates regional offices across the state, and any of them can accept your Form DSS-EA-270 application, verify documents, and answer questions about QMB, SLMB, or QI eligibility.

Here's how to find the right office — and what to bring when you go.

Major DSS Office Locations

South Dakota DSS maintains offices in the state's population centers. All offices are part of the Division of Economic Assistance and apply the same statewide eligibility rules — there's no variation in policy between locations.

Sioux Falls — serves the eastern region and the state's largest metro area. This office handles the highest volume of MSP applications in the state.

Rapid City — serves the western region, including families in the Black Hills area and those traveling from Pine Ridge and other western reservations.

Aberdeen — serves the northeastern region, covering Brown County and surrounding rural counties.

Pierre — the state capital office, which also houses the Division of Economic Assistance headquarters and the central phone line at 605-773-3165.

Additional DSS staff are available in smaller communities across the state. Call the central number at 1-800-305-3064 to find the nearest location if you're in a rural county far from these four offices.

What to Bring to Your DSS Visit

Whether you're filing a new application or delivering verification documents for a pending case, bring:

  • Medicare card showing your Medicare Beneficiary Identifier (MBI) and Part A/B effective dates
  • Social Security award letter or benefit verification letter showing your gross monthly benefit amount (not the net deposit — DSS needs the gross figure before the Part B deduction)
  • Three months of bank statements for every checking, savings, and investment account
  • Pension or VA benefit letters if you receive any income beyond Social Security
  • South Dakota ID or driver's license
  • Authorized-representative sections of Form 270 if an adult child will manage the case; attach POA or guardianship papers only if signing in that legal capacity

The complete document checklist is in the South Dakota Medicare Savings guide, which walks through each verification item and explains what DSS is looking for.

Online and Phone Alternatives

You don't have to visit in person. South Dakota offers three application channels:

  1. Online — through the Economic Assistance Portal at eaportal.sd.gov, where you can create an account, fill out the application, and upload scanned documents
  2. Mail — download Form DSS-EA-270, complete it on paper, and mail it with copies of your verification documents to your regional DSS office
  3. Phone — call 1-800-305-3064 to complete an application by phone, with follow-up verification submitted by mail or fax

For families in rural counties where the nearest DSS office is 50 or 100 miles away, the phone or online options save a significant trip. The 45-calendar-day processing deadline runs from the application submission date, regardless of which channel you use.

Tips for an In-Person Visit

If you do visit a DSS office, a few things to know:

  • Bring originals and copies. DSS workers can make copies of your documents, but having your own copies ready speeds up the process.
  • Ask for a receipt. Get written confirmation that your application was received and its submission date — this matters for the 45-calendar-day processing deadline and for retroactive SLMB/QI benefits.
  • Name your authorized representative. If an adult child will be managing the case going forward, complete Sections 4 and 17/18 of Form 270. This designates the person DSS may discuss the case with; attach POA or guardianship papers if they are signing in that legal capacity.

The full application walkthrough and eligibility worksheet covers every section of Form 270 and the income calculation that determines which program level — QMB, SLMB, or QI — applies to your situation.
