South Carolina Extra Help Medicare: Prescription Drug Assistance for Seniors
Prescription costs on Medicare add up fast without help — the standard Part D plan has a $590 deductible in 2026 and 25% coinsurance after that. Extra Help (officially the Part D Low-Income Subsidy) wipes out the deductible and caps what your parent pays per prescription. For South Carolina seniors, the fastest path to Extra Help runs through the Medicare Savings Programs.
The Automatic Route: MSP Triggers Extra Help
If your parent is approved for any Medicare Savings Program in South Carolina — QMB, SLMB, or QI — they're automatically "deemed" eligible for full Extra Help. No separate application to Social Security required. CMS sends a purple letter confirming the enrollment.
This deemed status locks in for the rest of the calendar year. Even if MSP status changes during the year, Extra Help continues through December 31.
What Extra Help Covers in 2026
The Inflation Reduction Act eliminated the old distinction between partial and full Extra Help. In 2026, everyone who qualifies gets the full benefit:
- Part D premium: $0 up to the regional benchmark plan amount
- Annual deductible: $0 (vs. $590 without Extra Help)
- Generic copay: Maximum $5.10 per prescription
- Brand-name copay: Maximum $12.65 per prescription
Combined with the 2026 Part D redesign that caps total out-of-pocket drug costs at $2,100 annually, Extra Help means your parent's prescription burden drops dramatically.
Applying Directly Through Social Security
If your parent doesn't qualify for an MSP or hasn't applied for one yet, they can apply for Extra Help directly through Social Security. The income and asset limits for standalone Extra Help are higher than the MSP limits — up to 150% FPL with asset limits of $18,090 for an individual and $36,100 for a couple.
But the MSP route is better when possible: it covers the Part B premium too, and the Extra Help comes automatically. An Extra Help application filed at SSA also generates a "lead" that SCDHHS receives electronically — the state sends an outreach letter and simplified verification request. If your parent gets this notice, treat it as an active MSP application and respond with asset documentation promptly.

What Happened to SC GAPS
The South Carolina Guaranteed Access to Prescription Assistance (SC GAPS) program is permanently closed. Seniors who previously relied on GAPS for medication cost help have no state-level prescription program to fall back on — Extra Help through an MSP is the replacement pathway. Any website still listing GAPS as an option is outdated.
Getting Started
The fastest way to cut your parent's prescription costs is applying for a Medicare Savings Program with SCDHHS, which triggers Extra Help automatically. The South Carolina Medicare Savings Programs & Extra Help Guide covers both the MSP application and how to confirm Extra Help is active on your parent's Part D plan.
