How to Become a Qualified Service Provider (QSP) in North Dakota

If you want to get paid for providing home care in North Dakota — whether for a family member or as a career — you must enroll as a Qualified Service Provider through the state's QSP registry. Every dollar of state or federal reimbursement through SPED, Ex-SPED, MSP-PC, or the Medicaid HCBS Waiver flows only to enrolled QSPs who have cleared the full onboarding pipeline.

The process is more structured than most families expect. Here is exactly what it takes.

TrainND Registration and the $10 Processing Fee

Your first step is submitting a referral form to TrainND Northeast, operated through Lake Region State College in Devils Lake. You will pay a $10 nonrefundable processing fee with your application.

TrainND assigns a registered nurse trainer who provides two comprehensive workbooks covering personal care techniques, safety protocols, and documentation requirements. You complete the self-study chapters at your own pace, then schedule an in-person practical evaluation with your assigned nurse. This hands-on competency check runs one to four hours depending on your prior experience.

When you pass, the nurse completes Form SFN 750 (Documentation of Competency) and routes it directly to TrainND, which forwards it to the state. You do not submit this form yourself.

Administrative Credentials You Need Before Enrollment

After clearing competency, you must gather three additional credentials before the state will activate your QSP status:

National Provider Identifier (NPI): Apply through the CMS National Plan and Provider Enumeration System. Processing typically takes seven to ten business days. You need this number to submit any claims.

Fraud, Waste, and Abuse (FWA) training: Complete the state's interactive online FWA module. This is a federal compliance requirement — you cannot skip it regardless of prior healthcare experience.

Direct deposit and identity documents: Upload proof of identity, a voided check or bank direct deposit form, and your NPI through the state's QSP Enrollment Portal.

Background Check Requirements

Every QSP applicant undergoes a state and federal background check before enrollment is finalized. The check covers criminal history, the sex offender registry, and the Office of Inspector General exclusion list. Processing times vary, but plan for two to four weeks.

If you are hiring a non-family caregiver as an individual QSP, the same background check applies to them. Agency QSPs conduct their own employee screening, but the state still verifies the agency's enrollment credentials.

The Therap Billing Transition (Critical for 2026)

As of July 1, 2026, ND HHS denies electronic claims submitted through the legacy MMIS web portal. All QSPs must now bill through Therap, a specialized online platform that handles electronic visit verification, service documentation, and claim submission.

Setting up a Therap account takes up to three weeks. If you miss this window, your care payments stop — there is no grace period for electronic claims.

Paper billing on Form SFN 1730 is still technically permitted, but processing delays can stretch weeks beyond electronic submissions. Therap is effectively mandatory for anyone who needs reliable, timely reimbursement.

Timeline: What to Expect End to End

Most individual QSP enrollments take six to ten weeks from initial TrainND registration to first billable service. The bottlenecks are the competency evaluation scheduling (TrainND nurse availability in rural areas), NPI processing, and background check turnaround.

If you are enrolling to care for a family member through one of the paid caregiver programs, start the QSP process immediately — do not wait until your parent's care plan is finalized. The 30-day service activation window after SPED or waiver pool clearance will expire if you are not yet enrolled.

Provider Moratorium: Burleigh and Cass Counties

A temporary enrollment moratorium effective June 11, 2026 blocks new QSP Agency and DD Agency enrollment in Burleigh County (Bismarck) and Cass County (Fargo). Individual QSP enrollment is not affected — you can still enroll as a solo provider in any county. But if you were planning to start a home care agency in the state's two most populous areas, that pathway is currently closed.
