QMB Processing Time in New Jersey: How Long It Takes and When Benefits Start

The Wait After You Hit Submit

Your parent's NJSave application is in. The documents are uploaded. Now the question that keeps you checking: how long until the Part B premium stops coming out of their Social Security check?

The honest answer involves two separate timelines, and confusing them is the most common source of panic during the wait.

The 45-Day Processing Standard

New Jersey state agencies are subject to a 45-day processing standard for Medicare Savings Program determinations. That standard runs from the agency's receipt of the application — either through the NJSave portal or at the county Board of Social Services. If the agency requests additional documentation (bank statements, a pension letter, proof of residency), respond promptly so the review can continue.

In practice, straightforward QMB applications submitted through NJSave with all documents attached often process within 30 to 45 days. Applications that route to the county welfare agency for additional asset verification — particularly those involving joint bank accounts or retirement account distributions — can stretch to 60 days or longer, especially in high-volume counties like Essex, Bergen, and Hudson.

When Coverage Actually Starts

QMB coverage is strictly prospective under federal law. Benefits begin on the first day of the calendar month following the month of approval. If your parent is approved on March 15, QMB coverage starts April 1. There is no retroactive QMB coverage — the federal statute does not allow it.

This matters because your parent continues paying the full Part B premium ($202.90/month in 2026) for every month before the effective date, including the months the application was being processed.

SLMB and QI are different. Those benefits can be effective retroactively up to three calendar months before the application month, as long as your parent was financially eligible during those months.

The Second Wait: Part B Premium Refund

Here's where families get confused. Even after QMB is approved and coverage starts, the Part B premium doesn't stop coming out of your parent's Social Security check right away.

After approval, New Jersey transmits the buy-in data to the Social Security Administration through the BENDEX electronic system. SSA then updates its payment files to stop deducting the Part B premium. This process takes 60 to 90 days. During that lag, your parent's Social Security check continues to be reduced by $202.90 each month.

Once SSA processes the update, two things happen:

  1. The monthly Part B premium deduction stops going forward
  2. SSA issues a retroactive reimbursement check for all premiums withheld between the QMB effective date and the date SSA updated its files

So if QMB starts April 1 and SSA processes the buy-in in late June, your parent gets a lump-sum refund for April, May, and the portion of June — roughly $400 to $600.

What to Do If the Wait Goes Past 45 Days

If your parent's application has been pending for more than 45 days with no decision and no request for additional documents, call the NJSave helpline at 1-800-792-9745 and ask for a status update. Get the name of the caseworker and the case number.

If the application is stuck at the county welfare agency, you can also contact your parent's county Board of Social Services directly. In persistent cases, Legal Services of New Jersey (1-888-LSNJ-LAW) can intervene on processing delays.

The New Jersey Medicare Savings Programs Guide includes an application progress tracker with dated fields for each milestone — submission, document requests, approval date, buy-in transmission, and premium refund — so you can see exactly where your parent's case stands at any point in the process.
