QMB in New Jersey: Eligibility, Application, and What It Covers

QMB Is the Most Comprehensive Medicare Savings Program

QMB — Qualified Medicare Beneficiary — is the top tier of New Jersey's Medicare Savings Programs, and it covers far more than the Part B premium. If your parent qualifies, QMB eliminates virtually every out-of-pocket Medicare cost: premiums, deductibles, coinsurance, and copays. Providers who bill a QMB member for cost-sharing are violating federal law.

For a senior on Original Medicare, QMB can save $7,000 to $10,000 or more per year depending on medical usage — the Part B premium alone is $2,434.80 annually in 2026.

2026 Eligibility in New Jersey

Requirement Single Married Couple
Monthly income Up to $1,330 Up to $1,803.33
Annual income Up to $15,960 Up to $21,640
Asset limit $9,950 $14,910

Income is gross Social Security minus the standard $20 unearned income disregard. A parent whose gross Social Security is $1,345/month has countable income of $1,325 — under the QMB threshold.

Assets include bank accounts, CDs, stocks, and cash-value life insurance. The home, one car, irrevocable burial trusts, and up to $1,500 in burial funds are excluded. No five-year lookback applies.

What QMB Actually Covers

  • Part B premium: $202.90/month, paid by the state
  • Part A premium: Paid in full if your parent doesn't have premium-free Part A (up to $565/month in 2026)
  • Part A inpatient deductible: $1,736 per benefit period
  • Part B annual deductible: $283
  • All Medicare coinsurance and copays: Hospital daily charges, skilled nursing, 20% Part B coinsurance — all covered
  • Automatic Extra Help: Full LIS for Part D drugs. QMB-only members generally pay up to $5.10 for generics and $12.65 for brand-name drugs in 2026; full-benefit duals with income at or below 100% FPL pay up to $1.60 and $4.90, respectively.

How to Apply in New Jersey

QMB applications go through NJSave, the state's combined benefits portal. After submission, eligibility is determined through DoAS/DMAHS processing and, in some cases, the county Board of Social Services.

The processing standard is 45 days, though county-level processing can take longer. QMB coverage is strictly prospective — it starts the first day of the month following approval. There's no retroactive coverage, which means applying sooner matters.

SLMB and QI, by contrast, can be applied retroactively up to three months.

Billing Protections Are Law

Federal law (42 U.S.C. § 1396a(n)(3)(B)) prohibits all Medicare-enrolled providers from billing QMB patients for deductibles, coinsurance, or copays. This applies to every doctor, hospital, and lab that accepts Medicare — even those not enrolled in NJ Medicaid.

New Jersey Medicaid reimburses providers under a "lesser-of" policy that often results in a $0 payment for cost-sharing. That $0 does not authorize the provider to bill your parent. The unpaid balance must be written off.

If your parent receives a bill despite QMB status, the escalation path starts with a written notice to the billing department citing the law, then a complaint to 1-800-MEDICARE, then a report to NJ DMAHS.

Estate Recovery Does Not Apply

QMB cost-sharing payments are explicitly exempt from Medicaid estate recovery under federal law. New Jersey does not seek recovery for QMB benefits themselves; other Medicaid coverage is subject to separate recovery rules.

The New Jersey Medicare Savings Programs, Extra Help & PAAD/Senior Gold Guide walks through the QMB application step by step, including the NJSave document checklist, billing dispute tools, and what to expect after approval.
