PCS Program in Nevada: Medicaid Personal Care Services Explained

Unlike Nevada's Frail Elderly Waiver, which has capped enrollment and a waitlist, Medicaid Personal Care Services (PCS) is an entitlement benefit — if your parent qualifies, they get services. No waitlist, no lottery.

That distinction matters. Families who spend months waiting for a waiver slot often do not realize PCS could have covered their parent's basic daily care needs from the start.

What PCS Covers

PCS provides hands-on help with activities of daily living (ADLs) for people with chronic health conditions or disabilities. Covered services include:

• Bathing, dressing, and grooming assistance
• Toileting and continence care
• Transfers (bed to wheelchair, wheelchair to car)
• Light housekeeping directly related to the person's care
• Meal preparation
• Mobility assistance within the home

PCS does not cover skilled nursing tasks like wound care or medication management under the standard agency model. However, the self-directed model opens a pathway to authorized skilled tasks (more on that below).

Eligibility Requirements

Your parent must qualify under Nevada's Medical Assistance for the Aged, Blind, and Disabled (MAABD) program:

• Income limit: $994/month for a single applicant
• Asset limit: $2,000 in countable assets
• Functional need: Must demonstrate a need for hands-on help with ADLs (a clinical assessment confirms this)

The financial application goes through DWSS. The functional assessment is conducted by ADSD or a contracted assessor.

If your parent's income exceeds $994/month but is below $2,982/month, they may still qualify for Medicaid under long-term care rules — particularly through the Frail Elderly Waiver, which uses different income thresholds and allows a Miller Trust for excess income.

Two Service Delivery Models

Agency-Directed (Provider Type 30): A licensed home care agency employs the caregiver. The agency handles hiring, scheduling, training, and supervision. Your parent has limited say in who provides the care.

Self-Directed ISO Model (Provider Type 83): Your parent (or their designated representative) acts as the managing employer. They recruit, train, schedule, and supervise their own caregiver. An Intermediary Service Organization (ISO) handles payroll, tax withholding, and background checks.

The self-directed model allows hiring adult children, siblings, grandchildren, or friends as paid caregivers through Medicaid. The restriction: legally responsible adults — spouses, court-appointed guardians, or parents of a minor — cannot be paid under this program.

A major advantage of the self-directed model is the Self-Directed Skilled Care Program under NRS 629.091. With a physician's annual certification, a self-directed caregiver can perform specific medical tasks including catheter care, wound dressing, oxygen management, and tube feedings.

How to Apply

1. Contact Nevada Care Connection (1-866-761-6263) for initial screening
2. File a Medicaid application through DWSS (online, by mail, or in person)
3. Complete the clinical functional assessment through ADSD
4. If approved, choose between agency-directed and self-directed service delivery
5. Select your agency or ISO provider

Processing typically takes 45 to 90 days depending on documentation completeness and assessment scheduling availability.
