Ohio Medicare Savings Program Application Forms: ODM 07216 vs JFS 07103
Three Forms, One Program
Ohio uses multiple application forms that all lead to the same Medicare Savings Program (MPAP) determination. Which one to use depends on what else you're applying for and how your county JFS handles intake.
Form ODM 07216 — Application for Health Coverage & Help Paying Costs. This is the primary state form, administered by the Ohio Department of Medicaid. It's 27 pages and covers standalone MPAP eligibility. If the applicant is aged, blind, or disabled and applying only for Medicare premium help, they'll need to complete Appendix E for the non-MAGI eligibility determination. This is the form most county offices prefer and what the Ohio Benefits portal generates electronically.
Form JFS 07200 — Application for SNAP, Cash, Medical, or Child Care Assistance. Use this multi-program form when the applicant wants to apply for food assistance (SNAP) or cash benefits alongside medical coverage. It's administered by the Ohio Department of Job and Family Services and routes through the same county JFS processing system.
Form JFS 07103 — Medicare Premium Assistance Application. A simplified, shorter paper form accepted by specific county JFS offices — particularly rural and smaller counties like Ottawa and Columbiana. It's designed specifically for MPAP intake, cutting out the sections about other programs. Not every county accepts it, so check with your local office first.
How to Submit
All three forms go to the CDJFS in the applicant's county of residence. Submission options:
- Mail — Send the signed original to your county JFS address and keep proof of mailing. The filing date is based on when the county receives the application.
- Fax — Fax to your county JFS fax number. Keep the transmission confirmation and check with the county to confirm receipt.
- In person — Drop off at the county JFS office. Ask for a date-stamped receipt.
- Online — The Ohio Benefits portal at benefits.ohio.gov generates the ODM 07216 electronically. You'll need an OHID login.
Urban counties like Franklin, Cuyahoga, and Hamilton operate centralized document processing centers; intake methods vary by county. Rural counties may prefer the paper JFS 07103 dropped off or mailed.
Choosing the Right Form
For most applicants, ODM 07216 is the safest choice — it's the state's preferred form for standalone MPAP applications. If you're applying online, the portal uses this form automatically.
Use JFS 07200 if you're also applying for SNAP, cash, or child care assistance in the same submission. Filing one multi-program application is more efficient than filing two separate forms.
Use JFS 07103 only if your county office specifically offers and accepts it. Call your county JFS first to confirm. The advantage is a shorter, less intimidating paper form focused solely on Medicare premium assistance.
Regardless of which form you use, attach the same verification documents: Medicare card, Ohio residency proof, income documentation (Social Security statement, pension statements), and bank statements for the most recent month.
