Golden Buckeye Card vs. Medicare Savings Program in Ohio

Two Programs That Sound Similar but Do Very Different Things

Ohio seniors often hear about the Golden Buckeye card and the Medicare Savings Programs in the same breath — at senior centers, from OSHIIP counselors, in state mailings. Both are state programs for older Ohioans. That's where the similarity ends.

The Golden Buckeye card is a state discount card administered by the Ohio Department of Aging. The Medicare Savings Programs (QMB, SLMB, QI) are Medicaid-administered benefit programs that pay Medicare premiums. Mixing them up can cost your parent thousands of dollars in unclaimed benefits.

What the Golden Buckeye Card Actually Does

Any Ohio resident aged 60 or older, or an Ohio resident aged 18-59 with a permanent and total disability, can get a Golden Buckeye card for free. It provides point-of-sale discounts at participating retail businesses and pharmacies — a percentage off the posted price, similar to a store loyalty card.

What it does not do: pay Medicare Part B premiums, cover Medicare deductibles or coinsurance, provide prescription drug insurance, reduce Part D copays, or confer any Medicaid eligibility status. It is not insurance. It cannot be combined with Medicare Part D or Medicaid benefits to lower official copayments.

If your parent's primary concern is the $202.90 monthly Part B premium being deducted from their Social Security check, the Golden Buckeye card cannot help.

What the Medicare Savings Programs Do

The Medicare Savings Programs are a different category entirely. They are administered by Ohio Medicaid through county JFS offices and pay real Medicare costs:

  • QMB covers the Part B premium, Part A premium (if applicable), Part A and Part B deductibles, and all Medicare coinsurance and copays. Income limit: $1,350 per month single in 2026.
  • SLMB covers the Part B premium. Income limit: $1,616 per month single.
  • QI covers the Part B premium. Income limit: $1,816 per month single.

All three tiers automatically qualify your parent for full Extra Help with Part D prescription costs — $0 plan premium on benchmark plans, roughly $5.10 for generics and $12.65 for brand-name drugs.

Ohio Has No State Pharmacy Assistance Program

This is the other piece families miss. Some states run State Pharmacy Assistance Programs (SPAPs) that provide a second layer of drug coverage on top of Medicare Part D — Pennsylvania has PACE, New York has EPIC. Ohio does not operate an SPAP.

The Golden Buckeye card's pharmacy discounts are the closest Ohio gets to a state drug discount for seniors, and those discounts operate entirely outside the Medicare Part D framework. They cannot reduce the copays your parent owes under their Part D plan.

For Ohio Medicare beneficiaries, the primary path to affordable prescription costs is the federal Extra Help program (Part D Low-Income Subsidy). The fastest way to get Extra Help is through the Medicare Savings Programs — approval for QMB, SLMB, or QI automatically deems your parent eligible for full Extra Help without a separate SSA application.

The Practical Comparison

Golden Buckeye Medicare Savings Program
Pays Part B premium No Yes (all three tiers)
Covers Medicare deductibles No Yes (QMB only)
Provides Extra Help for drugs No Yes (automatic)
Income test No income test; Ohio residents age 60+ or age 18-59 with a permanent and total disability Yes (up to 135% FPL)
Application Free; manual enrollment requires proof of age and Ohio residency, plus disability for applicants age 18-59 County JFS, 45-day process
Estate recovery risk Not applicable Exempt since 2010

Your parent can and should have both. The Golden Buckeye card offers modest retail savings. The Medicare Savings Programs deliver substantial, ongoing financial relief. One does not replace or conflict with the other.

Our Ohio Medicare Savings Programs guide covers the full eligibility screening, application process, and Extra Help interaction — everything the Golden Buckeye card doesn't touch.
