Who Needs Nursing Home Care

How to Pay

Medicare (Limited)

Medicare covers skilled nursing care only after a qualifying 3-day hospital stay, and only for up to 100 days of rehabilitation — not for long-term custodial care. Days 1–20 are fully covered; days 21–100 require a daily copayment. After day 100, Medicare coverage ends.

QUEST Integration (Medicaid)

For families with limited income and assets, QUEST Integration covers 100% of skilled nursing care costs. The financial eligibility thresholds for 2026:

  • Income: $1,530/month (individual) — Hawaii uses the medically needy pathway, so even if income exceeds this limit, your parent can qualify by spending excess income on medical costs
  • Assets: $2,000 (individual), $3,000 (couple both applying)
  • Home equity exemption: Up to $1,130,000 if a spouse or dependent child lives there
  • Community spouse protections: The non-applicant spouse can retain up to $162,660 in countable assets (CSRA)

A Medicaid recipient in a nursing home must contribute nearly all monthly income toward care, retaining only a $75 monthly Personal Needs Allowance.

The 5-year look-back: Med-QUEST reviews 60 months of financial records. Transferring assets below fair market value during this window triggers a penalty period calculated by dividing the transfer amount by the state's divestment divisor of $8,850. During the penalty period, the family pays all care costs out of pocket.

Long-Term Care Insurance

If your parent purchased a long-term care insurance policy before needing care, it may cover nursing home stays. Check the policy's benefit triggers, daily/monthly limits, elimination period, and maximum lifetime benefit.

Private Pay

Without Medicaid or insurance, nursing home care at $15,000+ per month depletes most families' savings within 1–3 years. The critical planning question is: how long until assets reach the Medicaid threshold, and is the family prepared for that transition?
