NJ FamilyCare and Medicare: How New Jersey Medicaid Works for Elderly Beneficiaries
NJ FamilyCare Isn't Just for Families With Children
When most people hear "NJ FamilyCare," they think of the program that covers children, pregnant women, and low-income adults under 65. But NJ FamilyCare is New Jersey's entire Medicaid program — and it includes coverage categories specifically designed for elderly and disabled Medicare beneficiaries.
If your parent is 65 or older and has Medicare, NJ FamilyCare has several pathways that can reduce or eliminate their out-of-pocket costs. The trick is knowing which pathway fits their situation.
The Aged, Blind, and Disabled (ABD) Category
The ABD category of NJ FamilyCare provides full Medicaid coverage to elderly and disabled individuals who meet income and asset requirements. For someone already on Medicare, full Medicaid through ABD means:
- QMB pays Medicare's deductibles, coinsurance, and copays for people who qualify
- Medicaid can cover benefits outside Medicare, subject to NJ FamilyCare category and program rules
- Access to MLTSS for eligible home-based and nursing facility long-term care
- Automatic eligibility for full Extra Help, which reduces Part D premiums and cost sharing
ABD Medicaid and MSPs are separate eligibility pathways with different rules; the $9,950 single and $14,910 married asset limits are the 2026 MSP limits and should not be used to determine ABD Medicaid eligibility. Full Medicaid may involve a spend-down requirement if income exceeds the Medicaid standard but is under the medically needy income level.
How SSI Triggers Automatic Enrollment
New Jersey is a Section 1634 state. This means anyone who receives Supplemental Security Income (SSI) is automatically enrolled in full NJ FamilyCare ABD Medicaid without filing a separate state application. SSA makes the SSI determination, and New Jersey accepts it as a Medicaid determination.
Your parent's Part B premiums are automatically picked up through the state buy-in. Before relying on QMB billing protections, confirm that QMB status is recorded; full Medicaid eligibility and QMB are distinct program designations.

NJ FamilyCare vs. Medicare Savings Programs
These are not either/or programs. They layer:
- MSPs alone (QMB, SLMB, QI): Help with Medicare premiums and cost-sharing, but no long-term care coverage. No estate recovery on MSP-only benefits.
- Full NJ FamilyCare ABD alone: Full Medicaid benefits including long-term care. Subject to estate recovery for long-term care services received at age 55 or older.
- Both together: The most complete coverage. Medicare pays first, QMB eliminates cost-sharing, and Medicaid covers everything else.
A parent who starts with just QMB may later need full Medicaid if they require nursing home care or extensive home-based services. The application routes are different — MSPs go through NJSave, while full NJ FamilyCare ABD applications go through the county Board of Social Services — but the eligibility determination considers income and assets under similar rules.
The New Jersey Medicare Savings Programs Guide explains how MSPs and NJ FamilyCare ABD interact, when to apply for each, and how estate recovery rules differ between the two — so your family can make enrollment decisions based on your parent's actual care needs and financial situation.
