New Jersey Medicare Eligibility and Cost Help for Seniors

Medicare Eligibility Is Federal — The Help Paying for It Is State

Medicare eligibility doesn't vary by state. Most people qualify at 65 or, if under 65, after 24 months of Social Security disability benefits. People with ALS can qualify as soon as they begin receiving disability benefits, and people who meet Medicare's end-stage renal disease requirements can qualify at any age. The rules are the same whether they live in Newark or Nebraska. What does vary dramatically is the financial help available to pay for Medicare once they're enrolled.

New Jersey is one of the more generous states for Medicare cost assistance. Between federal and state programs, a low-to-moderate-income senior in New Jersey can potentially eliminate the Part B premium, slash prescription costs to single-digit copays, and receive utility bill credits — all through one application.

The Full Stack of NJ Medicare Cost-Help Programs

Here's what's available, from most comprehensive to most targeted:

QMB (Qualified Medicare Beneficiary) — Pays Part A and Part B premiums, eliminates all Medicare deductibles, coinsurance, and copays. Income up to $1,330/month single. Asset limit $9,950.

SLMB (Specified Low-Income Medicare Beneficiary) — Pays the Part B premium ($202.90/month in 2026). Income up to $1,596/month single. Asset limit $9,950.

QI (Qualifying Individual) — Pays the Part B premium. Income up to $1,795.50/month single. Asset limit $9,950. Funded by a capped block grant — first come, first served.

Extra Help (Low-Income Subsidy) — Federal program that eliminates Part D deductibles and caps drug copays. Automatically granted to anyone approved for QMB, SLMB, or QI.

PAAD — New Jersey's own prescription program. Caps drug copays at $5 generic / $7 brand. Income up to $54,943/year single. No asset test.

Senior Gold — For income between $54,943 and $64,943 single. $15 copay plus 50% of remaining cost. After qualifying out-of-pocket expenses exceed $2,000 for a single enrollee, covered prescription copays become a flat $15 for the rest of the eligibility period. No asset test.

Lifeline — $225 annual credit toward electric or gas bills. Tied to PAAD or SSI eligibility.

HAAAD — Up to $500 per hearing aid ($1,000 total). Tied to PAAD income limits.

How They Layer Together

A single NJSave application screens for all of these simultaneously. Programs stack: a parent approved for QMB automatically gets Extra Help and likely qualifies for PAAD. That combination eliminates the Part B premium, wipes out hospital and doctor copays, and caps every prescription at $5 or $7.

For a parent on $1,300/month Social Security, that stack saves roughly $5,000 to $8,000 a year depending on their medical and prescription usage.

The Application Is Simpler Than It Looks

One NJSave application covers everything. The Division of Aging Services routes each component to the right agency. Apply online, by paper, or by calling 1-800-792-9745.

The biggest documentation hurdle is bank statements for the MSP asset test. PAAD and Senior Gold don't require them — only QMB, SLMB, and QI do.

The New Jersey Medicare Savings Programs, Extra Help & PAAD/Senior Gold Guide maps out every program, which ones your parent qualifies for, and the exact steps to apply through NJSave.
