New Mexico Medicare Savings Program Renewal: How Recertification Works
The 12-Month Cycle
Medicare Savings Programs in New Mexico operate on a 12-month recertification cycle. The Income Support Division reviews each case annually to confirm the beneficiary still meets income requirements.
The good news: in many cases, renewals happen automatically without paperwork.
Ex Parte Renewals (Automatic)
ISD first attempts an ex parte renewal by checking electronic databases — SSA income records, state wage files, and other data sources. If the system can verify that income hasn't changed or still falls within program limits, the MSP is renewed automatically.
When an ex parte renewal succeeds, your parent doesn't receive any paperwork and doesn't need to do anything. Coverage continues uninterrupted.
Manual Renewal Packets
When the automated check can't confirm eligibility — income changed, records don't match, or the database doesn't have current information — ISD mails a renewal packet. Your parent (or their authorized representative) must complete, sign, and return the packet with updated income proof within 30 days.
This is where coverage gets lost unnecessarily. Seniors who've moved, who have vision problems, or who don't understand the packet's urgency may miss the deadline. The packet looks similar to other state mailings that don't require action, and the consequences of ignoring it aren't always clear on the first page.
If the packet isn't returned in time, ISD closes the case for procedural reasons — not because your parent is ineligible, but because they didn't respond.

The 90-Day Reinstatement Window
New Mexico provides a 90-day safety net after a procedural closure. If the renewal packet and verifications are submitted within 90 days of the case closing, ISD reopens the case and processes it retroactively. No new application is required.
After 90 days, the protection expires and your parent would need to submit a fresh application.
Social Security COLA and the Annual Squeeze
The trickiest renewal period is January through March. Social Security Cost-of-Living Adjustments take effect in January, raising gross benefit amounts. But the federal poverty guidelines that set MSP income thresholds aren't updated until later — typically April 1 for implementation purposes.
New Mexico applies a COLA disregard during this gap. The monthly COLA increase is excluded from the MSP income calculation from January 1 until the new poverty guidelines take effect. This prevents a small COLA from pushing someone over the limit during the months before the thresholds catch up.
Without the disregard, a $50 COLA in January could technically move someone from QMB to SLMB (or from QI to ineligible) for three months, only for the new FPL thresholds to put them back in range in April. The disregard eliminates that disruption.
WEP/GPO Repeal Impact on Renewals
The Social Security Fairness Act, signed January 5, 2025, repealed the Windfall Elimination Provision and Government Pension Offset for benefits payable from January 2024 onward. SSA began paying retroactive adjustments in February 2025, and implementation is complete.
The recurring monthly increase counts as unearned income and can shift beneficiaries between MSP tiers. A retired teacher whose Social Security went from $850 to $1,100 after WEP repeal might move from QMB to QI at their next renewal.
The retroactive lump sum is treated as income only in the month received. In subsequent months, any unspent amount converts to a resource — which is irrelevant under New Mexico's zero-asset rules.
Staying Ahead of Renewal
Report income changes to ISD within 10 days. If your parent receives a renewal packet, respond immediately — don't wait for the 30-day deadline. If coverage does lapse due to a missed packet, use the 90-day reinstatement window rather than starting over with a new application.
