Neuropathic Pain in Elderly Parents: What Caregivers Need to Know
Nerve Pain Doesn't Follow the Rules Your Parent Expects
Neuropathic pain — pain caused by damage to the nerves themselves rather than by inflammation or injury to a joint — confuses older adults and their caregivers because it doesn't behave like "normal" pain. Your parent might describe burning in their feet when nothing is hot, electric shocks shooting down a leg with no apparent trigger, or a sensation that their sock is bunched up when their foot is bare.
Common causes in older adults include diabetic peripheral neuropathy (affecting up to 50% of people with long-standing diabetes) and post-herpetic neuralgia (the nerve pain that lingers for months or years after a shingles outbreak). Other culprits include chemotherapy-induced neuropathy, spinal stenosis compressing nerve roots, and vitamin B12 deficiency — which is underdiagnosed in seniors because its symptoms (tingling, balance problems, cognitive fog) overlap with so many age-related conditions.
What makes neuropathic pain particularly hard to manage at home: it may not respond adequately to the standard painkillers that help some arthritis pain. Acetaminophen and NSAIDs may not address the neuropathic component well. Understanding this distinction can prevent weeks of futile over-the-counter experiments.
How Caregivers Can Identify Nerve Pain
Nerve pain has distinct characteristics that separate it from musculoskeletal pain:
- Burning or tingling that follows a nerve distribution (both feet, one side of the chest along a rib line, one hand)
- Allodynia — pain from stimuli that shouldn't hurt, like bedsheets touching the feet or a light breeze on the skin
- Shooting or electric sensations that come in waves, often without any movement trigger
- Numbness coexisting with pain — your parent might report that their feet feel both numb and burning simultaneously, which sounds contradictory but is a hallmark of neuropathy
- Worse at night — neuropathic pain frequently intensifies when external stimulation drops, which is why bedtime becomes a flashpoint
If your parent uses a 0–10 pain scale, the character of the pain matters more than the number. A score of 5 that burns and tingles needs a fundamentally different treatment approach than a score of 5 that aches and stiffens.
Medication Options and Their Elderly-Specific Risks
Neuropathic pain has its own pharmacological tier, and every first-line drug carries age-related cautions:
Gabapentin (Neurontin) and pregabalin (Lyrica) are commonly prescribed nerve pain medications. They dampen abnormal nerve signaling, but benefit and timing vary. The problem in older adults: both drugs can cause dose-dependent drowsiness, dizziness, and cognitive blunting. They clear through the kidneys, so any decline in renal function increases blood levels. Falls risk rises. If your parent starts gabapentin, watch closely for unsteadiness, confusion, or excessive sleepiness and ask the prescriber whether the dose needs adjustment.
Duloxetine (Cymbalta) is an SNRI antidepressant with FDA approval for diabetic neuropathy. It works on descending pain pathways in the spinal cord. It's often better tolerated than gabapentin for cognitive side effects, but it can cause nausea, dry mouth, and — importantly — should never be abruptly discontinued (withdrawal symptoms can be severe). It also interacts with many common drugs in elderly polypharmacy regimens.
Topical lidocaine patches (5%) provide local nerve-blocking with generally lower systemic exposure than oral medicines. For post-herpetic neuralgia, they're a commonly used option, but interaction and toxicity risks are not zero. Follow the product or clinician instructions for the number of patches and wear time.
Topical capsaicin (high-concentration 8% patches, applied in-clinic) can desensitize nerve endings and may provide relief for months after an application, though the initial burning is significant.
What to avoid: The Beers Criteria flags several drugs commonly prescribed for nerve pain as potentially inappropriate in older adults — including tricyclic antidepressants (amitriptyline), which increase fall risk, urinary retention, and cardiac arrhythmia risk in the elderly despite being effective for neuropathy at low doses.

Non-Drug Strategies That Actually Help Nerve Pain
Unlike arthritis, where movement and heat are reliable first-line tools, neuropathic pain requires a different non-drug toolkit:
- TENS units — transcutaneous electrical nerve stimulation can modulate pain signaling, and many patients with peripheral neuropathy report meaningful relief. Critical safety note: TENS is absolutely contraindicated if your parent has a pacemaker or implantable cardiac defibrillator. Verify cardiac device status before purchasing a TENS unit.
- Desensitisation therapy — for allodynia (painful sensitivity to touch), gradually exposing the affected area to progressively firmer textures (silk → cotton → terrycloth) can retrain the nervous system over weeks.
- Warm foot soaks — for diabetic neuropathy in the feet, warm (not hot — always test temperature with your own hand first) water soaks can temporarily ease burning. Never use a heating pad directly on neuropathic feet; sensation loss means burns happen without awareness.
- B12 supplementation — if blood levels are low, correcting B12 deficiency can stabilise or partially reverse neuropathy. Ask the doctor to check methylmalonic acid levels, which are more sensitive than serum B12 alone.
- Blood sugar optimisation — for diabetic neuropathy, maintaining blood sugar within the clinician's target is an important long-term intervention.
When to Escalate Beyond Primary Care
Some neuropathic pain presentations require specialist involvement:
- Rapid onset of numbness or weakness in both legs — could indicate spinal cord compression, which is a surgical emergency (decompression is needed within 24–48 hours)
- Neuropathy progressing despite medication — a neurologist can perform nerve conduction studies to determine whether the damage is advancing
- Pain that remains inadequately controlled despite prescribed treatment — a multidisciplinary pain clinic can consider additional approaches, including interventional options, when appropriate
- New neuropathy without an obvious cause — needs workup for treatable causes including B12 deficiency, thyroid disease, monoclonal gammopathy, and other reversible conditions
Tracking Nerve Pain for Better Doctor Visits
Neuropathic pain fluctuates in ways that are hard to describe from memory. A structured pain log that captures the character (burning, shooting, tingling), timing (worse at night, triggered by touch), and functional impact (sleep disrupted, unable to wear shoes) gives doctors the data they need to adjust treatment.
