Why Pain Medication Hits Older Adults Differently
The same dose of ibuprofen or oxycodone that works safely in a 45-year-old can cause serious harm in an 80-year-old — not because older adults are fragile, but because their bodies process drugs fundamentally differently.
Kidney function declines with age, meaning drugs that are cleared through the kidneys stay active longer and reach higher concentrations. Liver metabolism slows, extending the half-life of drugs processed hepatically. Body composition shifts — more fat, less water — which alters how fat-soluble drugs distribute. Together, these changes mean medications accumulate faster, peak higher, and clear slower than they would in a younger body.
Add polypharmacy — more than 40% of adults over 65 take five or more daily prescription medications — and the risk of drug-drug interactions, compounding side effects, and adverse events multiplies. A pain medication that's safe in isolation may become dangerous in the context of blood thinners, blood pressure medications, antidepressants, or sleep aids already in the regimen.
Acetaminophen: The Default — But Not Unlimited
Acetaminophen (Tylenol/paracetamol) is one non-opioid option clinicians may consider because it doesn't carry the same gastrointestinal bleeding or kidney risks as oral NSAIDs. But it isn't risk-free or suitable for everyone.
The standard geriatric maximum: 3,000 mg per 24 hours. Do not assume a general-adult limit applies; follow the patient's clinician or pharmacist instructions. The reduced ceiling accounts for age-related changes in liver function.
For high-risk patients: 2,000 mg per 24 hours. This lower ceiling applies if your parent has liver disease, a history of alcohol misuse, is malnourished, or is underweight. These conditions reduce the liver's capacity to safely metabolize acetaminophen.
The hidden-dose trap. Acetaminophen hides inside dozens of combination medications: Tylenol PM, Excedrin, Percocet, Vicodin, NyQuil, and many cold and flu remedies. If your parent takes Tylenol for pain and then takes a cold medicine that also contains acetaminophen, they can exceed the daily limit without either of you realizing it. Read every label. Add up every source.
NSAIDs: The Risks That Escalate with Age
Nonsteroidal anti-inflammatory drugs — ibuprofen (Advil/Motrin), naproxen (Aleve), aspirin in anti-inflammatory doses — are on the AGS Beers Criteria list of potentially inappropriate medications for older adults. The 2023 Beers Criteria specifically flags oral NSAIDs for seniors because the risks increase dramatically after 65:
Gastrointestinal bleeding. NSAIDs damage the stomach lining and increase the risk of ulcers and GI bleeds. This risk is highest in patients also taking blood thinners (warfarin, apixaban), corticosteroids, or SSRIs. A GI bleed in an older adult can be life-threatening.
Kidney damage. NSAIDs reduce blood flow to the kidneys. In older adults with existing kidney function decline (even mild, undiagnosed decline), regular NSAID use can push kidney function into dangerous territory.
Cardiovascular risk. Chronic NSAID use increases the risk of heart attack and stroke, particularly in patients with existing cardiovascular disease.
The topical alternative. Topical NSAIDs — such as diclofenac gel (Voltaren) applied directly to a painful joint — may have less systemic exposure than oral NSAIDs, but they are not automatically safe for every older adult. If your parent's doctor has concerns about oral NSAIDs, ask whether a topical option is safe for them.

Opioids: What Caregivers Need to Know
When chronic pain is severe and hasn't responded to non-opioid treatments, opioids may be part of the management plan. The 2022 CDC Clinical Practice Guideline doesn't prohibit opioids for older adults — it recommends that non-opioid options be tried first, with soft warnings at 50 MME/day (morphine milligram equivalents) and extra caution at 90 MME/day.
Physical dependence vs. addiction. These are medically distinct. Physical dependence — where the body adapts to the medication and withdrawal symptoms occur if it's stopped abruptly — develops in anyone who takes opioids consistently. It's a physiological adaptation, not a disorder. Addiction involves compulsive use despite harm, craving, and loss of control. Understanding this distinction helps caregivers manage prescribed opioids without unnecessary guilt or fear.
Side effects to monitor actively:
Constipation. A common opioid side effect that can persist while the medication continues. Ask the prescriber or pharmacist how to prevent and manage it; do not start a stool softener or laxative without checking the patient's conditions and other medicines.
Sedation and cognitive clouding. Drowsiness and cognitive clouding can occur, and some older adults remain significantly sedated. This increases fall risk, impairs driving ability, and can mimic or worsen cognitive decline; report significant sedation to the prescriber.
Respiratory depression. The most dangerous opioid effect. More common when opioids are combined with benzodiazepines (Valium, Xanax, Ativan), muscle relaxants, or alcohol. If your parent takes any of these alongside an opioid, the combination requires close monitoring.
Nausea. Ask the prescriber or pharmacist how to manage nausea after a new opioid or dose increase. Do not change how the medication is taken without their instructions.
Transdermal Patches: Special Handling Required
Fentanyl and buprenorphine patches deliver medication through the skin, but the replacement interval and instructions are product-specific. They're useful for some patients with swallowing difficulty or absorption issues, but they require careful handling:
- Follow the specific product instructions for placement and replacement, and rotate application sites as directed to help prevent skin irritation.
- Never cut a patch — this can cause a dangerous dose dump of the entire medication at once.
- Heat increases absorption. Heating pads, electric blankets, hot baths, and fever can all cause the patch to release medication faster than intended, risking overdose. If your parent uses heat therapy for pain, never apply it over or near a transdermal patch.
- Used patches still contain active medication. Fold them sticky-side-in and dispose of safely. Keep away from children, pets, and anyone other than the patient.
Running a Home Medication Safety Audit
Conduct a complete audit of your parent's medication regimen whenever a new medication is prescribed, when your parent uses multiple pharmacies, or when they take over-the-counter pain medications:
1. List everything. Prescription medications, over-the-counter drugs, vitamins, supplements, herbal products. Include eye drops, inhalers, patches, and creams. Everything that goes in or on the body.
2. Check for duplicate ingredients. Look for acetaminophen appearing in multiple products. Look for NSAIDs in combination cold/flu remedies. Look for sedating antihistamines (diphenhydramine) in "PM" formulations.
3. Cross-reference the Beers Criteria. The 2023 AGS Beers Criteria lists potentially inappropriate medications for older adults. Key pain-related drugs to flag: oral NSAIDs, skeletal muscle relaxants (cyclobenzaprine, methocarbamol), first-generation antihistamines, and benzodiazepines.
4. Bring the list to one pharmacist. If your parent fills prescriptions at multiple pharmacies, no single pharmacist sees the full picture. A comprehensive medication review by a single clinical pharmacist can identify drug-drug interactions that nobody has caught.
5. Ask about deprescribing. Some medications your parent has been taking for years may no longer be appropriate or necessary. A geriatrician or clinical pharmacist can identify candidates for careful, supervised discontinuation.
