Moving to Florida Medicare Savings Program: Interstate Transfer Guide

Your Old State's MSP Does Not Transfer to Florida

This catches nearly every family off guard: Medicare Savings Programs do not transfer between states. Because MSPs run through each state's Medicaid system, your parent needs a separate Florida DCF application; notify the former state Medicaid agency and request formal closure of the old case. There is no automatic transfer, no interstate portability, and no way to carry benefits from New York or Ohio or any other state into Florida.

When coverage ends in the old state:

  • The Part B premium deduction resumes in your parent's Social Security check — $202.90/month in 2026
  • QMB billing protections end — providers can bill deductibles and copays again
  • The old state stops its buy-in payments to CMS

The gap between losing coverage in one state and getting approved in Florida is the most financially painful part of an interstate move for seniors on MSP.

Extra Help Transfers — MSP Does Not

Here's the one bright spot: Extra Help (Part D Low-Income Subsidy) is a federal program administered by Social Security, not by the states. It transfers with your parent nationally. Prescription drug cost reductions continue without interruption during the move.

If your parent's Extra Help was triggered by MSP enrollment, it continues through December 31 of the year MSP eligibility ends. SSA/CMS can reassess eligibility for the next year, so check the next eligibility notice.

Filing With Florida DCF Immediately

Start the Florida MSP application as soon as your parent establishes residency. Every month without coverage is another $202.90 Part B premium deducted from their Social Security check.

Proof of Florida residency is required. Any of these work:

  • Florida driver's license or state ID
  • Signed lease agreement
  • Utility bill in your parent's name at the Florida address
  • Florida voter registration

If your parent moved in with an adult child, use a Florida residency document from the list above; the child's income and assets are not counted toward the parent's MSP eligibility. Seasonal residents who maintain primary legal domicile in another state are ineligible — Florida requires intent to remain permanently or indefinitely.

Application routes:

  • Online through MyACCESS at myaccess.myflfamilies.com
  • Paper Form CF-ES 2282 mailed to ACCESS Central Mail Center, P.O. Box 1770, Ocala, FL 34478-1770
  • In person at a DCF service center or ACCESS Community Partner

Bring everything from the old state's MSP file: the approval notice, income documentation, bank statements, and any other records. Florida DCF runs its own verification, but having documentation ready prevents delays from documentation requests.

The Coverage Gap and How to Handle It

DCF has 45 calendar days to process the application. SLMB can be approved retroactively for up to three months before the application month. QI can reach back up to three months too, but only within the same calendar year — this can partially or fully cover the gap if you file quickly after moving.

QMB does not have retroactive coverage. It starts on the first day of the month after DCF approval. This means the gap between the old state's termination and Florida QMB's start date is a real loss — your parent pays Part B premiums and faces cost-sharing for that period.

To minimize the gap:

  1. File the Florida application before or immediately upon arrival — the same week if possible
  2. Don't wait to close the old state's case first — the states don't coordinate, and waiting only widens the gap
  3. Apply for QI or SLMB simultaneously if income is near the border — these have retroactive coverage and provide the same Part B premium relief

After Florida DCF approves the application, SSA processes the new buy-in record. This state-to-SSA handoff can take multiple months. During that period, the Part B premium may continue to be deducted from your parent's Social Security check. SSA reimburses premiums withheld for approved coverage months after the buy-in record processes.

Coordinating With the Old State

Notify the old state's Medicaid office of the move. Some states terminate MSP automatically when they detect a new address or when Social Security updates the state of residence. Others require a formal notification.

If the old state hasn't terminated coverage by the time Florida approves, there can be a temporary overlap in buy-in records at CMS. This usually resolves itself — CMS processes the old state's termination and the new state's enrollment — but it can delay the premium refund if SSA gets conflicting records. Calling Social Security to confirm that the old state's buy-in has ended and Florida's has begun can catch these issues early.

Adult Children Relocating Parents to Florida

For out-of-state adult children moving a parent to Florida, the administrative burden is significant. You're establishing new residency, finding new healthcare providers, setting up a new DCF case, and managing the transition in benefits — all while your parent is adjusting to a new environment.

Set up authorized representative status with Florida DCF immediately (Form CF-ES 2505 or a Florida durable power of attorney). This lets you manage the application, receive notices, and handle documentation requests on your parent's behalf.

SHINE counselors (call 1-800-963-5337) can help orient you to Florida's Medicare landscape, including Medicaid managed care plans that interact with QMB coverage.

The Florida Medicare Savings Programs & Extra Help Guide covers the full interstate transition, including a timeline checklist for coordinating the old state's closure with Florida's approval.
