Florida Medicare Savings Program Application: Step-by-Step Filing Guide
Three Ways to File Your Application
Florida's Medicare Savings Program applications go through the Department of Children and Families (DCF), not Medicare or Social Security. You have three filing options.
Online through MyACCESS (myaccess.myflfamilies.com) is the fastest route. Create an account, go to "Apply for New Benefits," and complete the medical assistance section. The critical step: check the box asking for "Help paying Medicare premiums." Missing that checkbox is one of the most common reasons applications stall — DCF processes the application as a general Medicaid request instead of routing it to the Medicare Savings Program team.
Paper Form CF-ES 2282 is the dedicated Medicare Savings Program application. Mail completed forms to the ACCESS Central Mail Center at P.O. Box 1770, Ocala, FL 34478-1770, or fax them to DCF at 1-866-886-4342.
In person at any DCF Economic Self-Sufficiency service center. You can also get hands-on help at ACCESS Community Partner locations — Area Agencies on Aging, Aging and Disability Resource Centers, legal aid offices, and community health centers can assist with filling out the paperwork.
Whichever route you use, you'll also need to sign Form CF-ES 2064 (the rights and responsibilities acknowledgment). MyACCESS handles this electronically during the online application.
Documents to Gather Before You Start
DCF verifies eligibility through automated data matching with Social Security, bank databases, and employment records. In many cases, electronic verification is enough and DCF approves without requesting paper documents. But gathering these upfront prevents delays if DCF can't verify something electronically:
- Medicare card showing Part A and/or Part B enrollment dates
- Social Security benefit statement (SSA-1099 or the monthly benefit letter) showing the gross monthly amount before the Part B deduction
- Bank statements for every open account — checking, savings, CDs — for the application month
- Pension or annuity statements showing gross monthly amounts
- Proof of Florida residency: driver's license, state ID, utility bill, or lease
- Immigration documents if applicable (green card, work authorization)
If DCF's electronic checks flag a discrepancy, they'll send a Notice to Acknowledge / Pending Documentation Request giving you 10 days to submit the missing items. That deadline is strict — missing it results in a procedural denial, even if your parent clearly qualifies.
Applying for a Parent: Authorized Representative Setup
Adult children filing on behalf of a parent need to establish authorized representative status with DCF. Two options:
Form CF-ES 2505 (or the representative section within Form CF-ES 2282) designates you as your parent's authorized representative within the DCF system. This lets caseworkers discuss the case with you, send you notices, and accept document uploads from you.
A Florida Durable Power of Attorney with health-care and public-benefits management provisions works in place of Form CF-ES 2505.
One thing to keep in mind: the DCF authorization covers only the DCF system. To talk to Social Security on your parent's behalf, you need Form SSA-1696. For Medicare questions through 1-800-MEDICARE, you need Form CMS-10106. These are separate authorizations — filing one doesn't cover the others.

What Happens After You Submit
DCF is legally required to process applications within 45 calendar days from the date it receives a signed application.
The typical sequence: DCF runs automated data matching against Social Security records and the Asset Verification System. If everything checks out electronically, they issue an approval Notice of Case Action. If they need additional documentation, they send the pending notice and pause the clock until you respond.
Track your application status through the MyACCESS portal dashboard or by calling the DCF Customer Call Center at 1-866-762-2237.
If 45 days pass without a written determination, you can request a supervisor review at your local service center or file a petition for a fair hearing based on agency delay.
Coverage Start Dates Vary by Program
This is where timing matters:
- QMB coverage starts on the first day of the month after DCF approves the application. No retroactive coverage.
- SLMB coverage can reach back up to three months before the month you applied, as long as your parent was eligible during those months; QI can reach back up to three months only within the same calendar year as the application.
After approval, DCF transmits a buy-in record to the Social Security Administration. SSA stops the Part B premium deduction after the record processes and reimburses premiums withheld for approved coverage months. This state-to-SSA handoff can take multiple months, so your parent may still see the deduction temporarily.
