Missouri QMB Application and Eligibility Requirements

What QMB Covers

Qualified Medicare Beneficiary (QMB) is the most comprehensive tier of Missouri's Medicare Savings Programs. It pays:

  • Part A premiums (relevant if you don't have premium-free Part A)
  • Part B monthly premium ($202.90/month in 2026 for most people)
  • Part A hospital inpatient deductible ($1,676 per benefit period)
  • Part B annual deductible
  • All coinsurance and copayments on Medicare-covered services

QMB also gives you federal billing protections under 42 CFR 447.15. Doctors, hospitals, and other providers are prohibited from balance-billing you for Medicare cost-sharing. If a provider sends you a bill for a deductible or coinsurance that QMB should cover, that bill is illegal.

Income and Asset Limits

To qualify for QMB in Missouri, your countable monthly income must fall at or below 100% of the federal poverty level:

  • Individual: $1,255/month after the standard $20 unearned income disregard
  • Couple: $1,703/month after the $20 disregard

FSD calculates your income from your gross Social Security benefit — the amount before Part B premiums, tax withholdings, or garnishments are deducted. They add back everything that was subtracted from your check, then apply the $20 disregard.

Resource limits are $9,660 for an individual and $14,470 for a couple. Your home and primary vehicle are fully excluded. Burial plots, up to $1,500 in designated burial funds, and term life insurance are also excluded. Retirement accounts in periodic payout status are excluded as assets (the payments count as income instead).

Joint bank accounts carry a presumption that the full balance belongs to the applicant. If someone else contributed to the account, you'll need bank records or documentation proving their share.

How to Apply

Submit Form IM-1MSP (Application for Medicare Savings Programs) to FSD through any of these channels:

  • Online: myDSS portal at mydss.mo.gov
  • Phone: 855-373-4636
  • Fax: 855-600-3754
  • In person: Any FSD Resource Center

Along with the form, you'll need to provide identity documents, proof of Missouri residency, your SSA-1099 or Social Security award letter, bank statements for all accounts, and documentation for any countable assets.

Processing takes up to 45 days under federal standards, though delays happen. Track your case status through myDSS or call the FSD Information Center.

When QMB Coverage Starts

QMB coverage is strictly prospective. It begins on the first day of the month after FSD approves your application — there's no retroactive coverage. This is different from SLMB and QI-1, which allow up to three calendar months of retroactive coverage.

This means timing matters. If you know your parent qualifies, file as early as possible. Every month of delay is a month of premiums, deductibles, and copays paid out of pocket that won't be refunded.

The Conditional Part A Complication

Missouri is a "group payer" state. If you or your parent don't have premium-free Part A (typically because of fewer than 40 quarters of work history), you can't just apply for QMB and get Part A thrown in. You must first file a conditional Part A enrollment at your local Social Security office during the General Enrollment Period (January 1–March 31), using specific language that prevents SSA from billing you directly.

This is a sequential process — the SSA filing has to happen before FSD can approve QMB for someone without Part A. Missing the GEP window means waiting until the next January.

For the exact script to use at the SSA office, document checklists, and eligibility worksheets, the Missouri Medicare Savings guide walks through every step of both the SSA conditional filing and the FSD application.
