Missouri Medicaid Renewal for Medicare Savings Programs
What Arrives in the Mail
Once a year, the Family Support Division (FSD) mails a renewal packet in a distinctive yellow envelope. Inside is Form FA-402 — the MO HealthNet Annual Renewal for non-MAGI programs. This is the form that keeps your QMB, SLMB, or QI-1 coverage running.
You typically have 30 days to complete and return it with updated financial verification.
What You Need to Submit
The renewal asks FSD to re-verify the same financial picture they evaluated at your initial application:
- Current gross Social Security benefit (the award letter or SSA-1099, not the net bank deposit)
- Any pension, VA benefit, or other unearned income documentation
- Bank statements for all accounts — all pages, not just the summary page
- Life insurance policy documents if you hold whole life insurance
- Retirement account statements if they've changed since the last review
- Any changes in household composition, address, or marital status
FSD evaluates your income using the same $20 monthly unearned income disregard they applied at enrollment. If your income went up due to a Social Security COLA increase but the new poverty guidelines haven't taken effect yet (January through March), the COLA disregard protects you from being dropped during that gap.
What Happens If You Miss the Deadline
If the FA-402 isn't returned in time, FSD closes your case for "failure to cooperate." Your MSP coverage ends, and with it your automatic Extra Help qualification and MoRx enrollment.
But there's a 90-day grace period. Within 90 days of closure, you can submit the missing renewal form and financial documents to get reinstated prospectively — without filing an entirely new application. After 90 days, you start over from scratch.

Reporting Changes Mid-Year
You don't wait for the annual renewal to report changes. FSD requires notification within 10 calendar days of any change in income, assets, address, or household composition using Form IM-145 (Change Report). You can also report changes through the myDSS online portal or by calling 855-373-4636.
This matters most when income increases. If a Social Security COLA, pension adjustment, or inheritance pushes you above your program tier's limit, FSD may shift you to a higher tier (from QMB to SLMB, for example) or close your case. Reporting promptly avoids overpayment issues that can complicate future applications.
QI-1 Renewals Work Differently
QI-1 is funded through a federal block grant with capped allocation, unlike QMB and SLMB which are open-ended entitlements. FSD prioritizes existing QI-1 enrollees during renewal. Return the renewal within the stated timeframe so FSD can review your continued eligibility.
Keeping Coverage Through the Renewal
The single most common reason people lose Medicare Savings Program benefits in Missouri is a missed renewal — not an income change. Set a calendar reminder to watch for the yellow envelope, and return the form well before the deadline.
If you're managing this for a parent, make sure Form IM-6AR (Authorized Representative) is on file so FSD can send you copies of notices and discuss the case with you directly.
The Missouri Medicare Savings guide includes a renewal preparation checklist that tracks exactly which documents FSD needs and when, so nothing falls through the cracks between annual reviews.
