Medicare Savings Program South Carolina: QMB, SLMB and QI Explained
That $202.90 Part B premium hitting your parent's Social Security check every month adds up to $2,434.80 a year — money a fixed-income household in South Carolina can't afford to lose. The state's Medicare Savings Programs exist specifically to eliminate that deduction, and depending on which tier your parent qualifies for, the help goes well beyond the premium.
The Three Tiers: QMB, SLMB, and QI
South Carolina runs three Medicare Savings Programs through the Department of Health and Human Services (SCDHHS), branded as Healthy Connections Medicaid. Each tier has different income ceilings and covers different costs.
Qualified Medicare Beneficiary (QMB) is the most comprehensive. It pays Part A and Part B premiums, the Part B annual deductible ($283 in 2026), and all Medicare coinsurance and copayments. For someone facing a $1,736 hospital deductible per benefit period, QMB coverage is substantial.
Specified Low-Income Medicare Beneficiary (SLMB) covers only the Part B premium — that $202.90 monthly deduction from Social Security. No cost-sharing help beyond the premium.
Qualifying Individual (QI) also covers only the Part B premium, but it operates on a capped federal block grant. South Carolina's internal code for QI is "SLMB II" or "SLMB-2," which can cause confusion on state paperwork. QI is allocated first-come, first-served, with priority going to people who received it the previous year.
One critical distinction: QMB-Only enrollees get a Healthy Connections Medicaid card, but it signals providers that cost-sharing is covered by the state — it doesn't provide full Medicaid benefits like dental or vision. Beneficiaries who independently qualify for full South Carolina Medicaid alongside QMB (called "QMB-Plus") get both.
Who Qualifies in 2026
Eligibility depends on monthly income (after a standard $20 disregard) and countable assets:
| Tier | Individual Income | Couple Income | Assets (Individual/Couple) |
|---|---|---|---|
| QMB | Up to $1,350 | Up to $1,824 | $9,950 / $14,910 |
| SLMB | $1,351–$1,616 | $1,825–$2,184 | $9,950 / $14,910 |
| QI | $1,617–$1,816 | $2,185–$2,455 | $9,950 / $14,910 |
The $20 general income disregard means someone with $1,370 in gross monthly income actually has $1,350 in countable income — right at the QMB ceiling. Your parent's home, one vehicle, and up to $1,500 set aside for burial are excluded from the asset count.
How to Apply
Applications go through SCDHHS using Form 3400 ("Application for Medicaid and Affordable Health Coverage"). Five submission routes: the Healthy Connections online portal at apply.scdhhs.gov, fax to (888) 820-1204, email to [email protected], mail to SCDHHS Central Mail at P.O. Box 100101 in Columbia, or in-person drop-off at any county eligibility office.
Processing takes 30 to 45 days. QMB coverage starts the first of the month after approval. SLMB and QI can be retroactive up to three months if your parent was eligible during those months.

The Extra Help Bonus
Every MSP tier — QMB, SLMB, and QI — automatically qualifies the beneficiary for Medicare Part D Extra Help. No separate application needed. CMS sends a purple letter confirming the enrollment. In 2026, Extra Help eliminates the Part D deductible entirely and caps drug copays at $5.10 for generics and $12.65 for brand-name medications.
What to Do Next
If your parent's Social Security check is losing $202.90 a month to the Part B premium and their income falls under the limits above, they likely qualify for at least one tier. The South Carolina Medicare Savings Programs & Extra Help Guide walks through the full application process, the income math with worked examples, and what to do if a provider tries to bill a QMB enrollee for cost-sharing they're not supposed to pay.
