How to Apply for Medicare Savings Program in South Carolina
Applying for a Medicare Savings Program in South Carolina means going through SCDHHS — the Department of Health and Human Services, which runs everything under the Healthy Connections Medicaid brand. The application itself is Form 3400, a multi-purpose Medicaid form. Here's how to navigate the parts that matter for Medicare premium help.
The Application: SCDHHS Form 3400
Form 3400 is titled "Application for Medicaid and Affordable Health Coverage." It covers multiple programs, including long-term care Medicaid and MSPs. You don't need to fill out the nursing home sections — only the pages covering Medicare status, income, and assets.
If your parent also needs home and community-based services, attach Form 3400-B. For retroactive coverage (SLMB and QI can go back up to three months), submit Form 3400-C alongside the main application.
Five Ways to Submit
Online: The Healthy Connections portal at apply.scdhhs.gov handles the full application digitally.
Fax: Send completed forms to (888) 820-1204. This goes directly to Central Mail processing — it doesn't route through a local office.
Email: Send the form as a PDF attachment to [email protected]. Same central processing destination as fax.
Mail: SCDHHS Central Mail, P.O. Box 100101, Columbia, SC 29202-3101.
In person: Drop off at any county SCDHHS eligibility office. Urban offices in Columbia, Charleston, and Greenville tend to have more availability; rural county offices may have limited hours.
For adult children managing this remotely, fax or email to Central Mail is the most efficient route. You skip the variable hours at county offices and get the same processing timeline.
Documents to Gather
SCDHHS will want verification, typically in hard copy:
- Copy of the Medicare card (the red, white, and blue card)
- Social Security award letter showing gross monthly benefit
- Recent pension or VA benefit statements
- Bank statements for checking, savings, and CDs (month of application and prior months)
- Life insurance policy details if total face value exceeds $1,500
- Prepaid burial contract documentation if claiming the burial exclusion
Missing documents don't kill the application — SCDHHS sends an Information Request Notice asking for what's missing. But the 45-day processing clock pauses until they receive it, so submitting everything upfront avoids delays.

Processing Timeline
Federal regulations give SCDHHS 45 days to process an MSP application that doesn't require a disability determination. In practice, South Carolina processes most within 30 to 45 days.
QMB coverage starts the first day of the month after approval — approve on March 15, coverage begins April 1. SLMB and QI can be retroactive up to three months from the application date if your parent was financially eligible during those months.
After Approval
Once approved, SCDHHS sends an electronic buy-in record to CMS, which coordinates with Social Security to stop the $202.90 Part B premium deduction. This handoff between agencies takes additional time — expect 60 to 90 days before the premium actually disappears from the Social Security check. Accumulated premiums paid after the coverage start date are refunded retroactively.
Your parent also automatically qualifies for Extra Help (Medicare Part D Low-Income Subsidy) through any MSP tier, with no separate application required.
The South Carolina Medicare Savings Programs & Extra Help Guide includes a document checklist, a section-by-section walkthrough of Form 3400 for MSP applicants, and a tracking timeline from submission through premium refund.
