Medi-Cal Rx Pharmacy Coverage for California Dual Eligible Beneficiaries

What Medi-Cal Rx Covers

Medi-Cal Rx is California's fee-for-service pharmacy benefit program, carved out of Medi-Cal managed care and administered directly by the Department of Health Care Services through Magellan Rx Management. For full dual eligibles who also have full-scope Medi-Cal, it acts as a secondary payer behind Medicare Part D. MSP-only beneficiaries are not eligible for Medi-Cal Rx.

Part D covers most outpatient prescription drugs for Medicare beneficiaries. For full-scope Medi-Cal duals, Medi-Cal Rx acts as a secondary payer for specific drug classes excluded from Part D by federal law, such as prescribed over-the-counter medications and certain vitamins.

California does not operate a separate State Pharmaceutical Assistance Program (SPAP). Low-income beneficiaries can receive Part D Extra Help (the Low-Income Subsidy), and full-scope Medi-Cal duals may also use Medi-Cal Rx for specific drug classes excluded from Part D by federal law. If your parent is enrolled in a Medicare Savings Program — QMB, SLMB, or QI — they automatically receive full Extra Help benefits, which reduce or eliminate Part D premiums, deductibles, and copays.

How the Two Programs Coordinate

When your parent fills a prescription, the pharmacy bills Part D first. If the drug is on the Part D plan's formulary, Part D pays. With full Extra Help, your parent's cost-sharing is minimal — copays are capped at a few dollars per prescription.

If Part D denies a claim because the drug is excluded from Part D by federal law, a full-scope Medi-Cal beneficiary can ask whether Medi-Cal Rx covers it. A formulary or prior-authorization denial by a Part D plan does not by itself mean Medi-Cal Rx will cover the drug. Medi-Cal Rx maintains its own formulary called the Contract Drug List.

The coordination happens at the pharmacy counter, but it does not always happen smoothly. Some pharmacies are not enrolled as Medi-Cal Rx providers, or their system does not automatically attempt the secondary claim. If your parent has full-scope Medi-Cal and is told a Part D-excluded drug is not covered, ask the pharmacist to check Medi-Cal Rx coverage specifically.

Pharmacy Network

Not every pharmacy participates in Medi-Cal Rx. While most chain pharmacies in California are enrolled, some independent pharmacies and specialty pharmacies are not. A full-scope Medi-Cal beneficiary needs to fill prescriptions at a pharmacy that accepts both their Part D plan and Medi-Cal Rx to get the full benefit of dual coverage.

The Medi-Cal Rx provider lookup on the DHCS website shows which pharmacies are enrolled. If your parent has full-scope Medi-Cal but uses a pharmacy that does not participate in Medi-Cal Rx, ask about an enrolled pharmacy before paying out of pocket for a Part D-excluded drug.

Common Confusion Points

"Do I need a Part D plan if I have Medi-Cal?" Yes. Medicare Part D or a Medicare Advantage Prescription Drug plan provides coverage for Part D drugs. For full-scope Medi-Cal duals, Medi-Cal Rx is a secondary benefit, not a replacement for Part D; MSP-only beneficiaries are not eligible for Medi-Cal Rx. MSP enrollment automatically confers deemed Extra Help; with Extra Help, benchmark-plan premiums are $0.

"My pharmacy says my drug needs prior authorization from Medi-Cal Rx." Ask the prescribing doctor or pharmacist how to submit the request and when to expect a decision. If the prior authorization is denied, your parent can appeal through the Medi-Cal Rx appeals process.

"My Part D plan changed my drug's tier or dropped it from the formulary." Contact the Part D plan and prescriber about the plan's transition rules, a formulary exception, or a therapeutic alternative. Medi-Cal Rx coverage is limited to specific drug classes excluded from Part D by federal law for full-scope Medi-Cal beneficiaries; a formulary change alone does not establish Medi-Cal Rx coverage.

Our California Medicare Savings Programs & Extra Help Guide explains how MSP enrollment triggers Extra Help, which Part D costs get eliminated, and how to coordinate Medi-Cal Rx when Part D falls short.
