LT-PCS vs Community Choices Waiver Louisiana: Which Program Fits?

Your parent qualifies for home-based Medicaid care in Louisiana. Two programs show up in every conversation with the Office of Aging and Adult Services — Long-Term Personal Care Services (LT-PCS) and the Community Choices Waiver (CCW). They sound similar, but they work very differently, and picking the wrong entry point can cost your family months of unnecessary waiting.

The Core Difference: Entitlement vs. Capped Slots

LT-PCS is a Medicaid State Plan service. It is an entitlement — if your parent meets the eligibility criteria, the state must provide it. There is no waitlist, no slot cap, and no Request for Services Registry to sit on.

The Community Choices Waiver is a Section 1915(c) home and community-based waiver with a hard cap on the number of funded slots. The state legislature controls how many slots are available. As of late 2025, roughly 8,053 funded CCW slots existed, with 11,274 people on the Request for Services Registry (RFSR) waitlist. New applicants who don't qualify for a priority group enter the general queue and wait — sometimes for years.

Eligibility Side by Side

Both programs require the applicant to meet the Nursing Facility Level of Care (NFLOC) standard, determined through the LOCET clinical assessment. The financial thresholds are where they split.

LT-PCS income limit: 100% of the Federal Benefit Rate — $994 per month in 2026. This is strict. If your parent's Social Security payment pushes them over $994, they do not qualify for LT-PCS through the standard pathway.

CCW income limit: $2,982 per month (the Special Income Limit, 300% of the Federal Benefit Rate). Parents with income between $994 and $2,982 qualify for the CCW's income bracket. Those over $2,982 can still qualify through the Medically Needy Spend-Down pathway, where excess income is offset by incurred medical expenses.

Both programs require countable assets under $2,000 for a single applicant. The standard Medicaid exempt assets apply — the primary home (up to $752,000 equity), one vehicle, household goods, and irrevocable burial contracts up to $10,000.

What Each Program Actually Provides

LT-PCS services are limited to agency-directed personal care. A home health agency sends personal care attendants to help with activities of daily living — bathing, dressing, grooming, meal preparation, and light housekeeping. The services are administered through the applicant's Healthy Louisiana managed care organization (MCO).

CCW services are broader. Beyond personal assistance, the waiver covers:

• Support coordination (a dedicated coordinator manages the entire care plan)
• Home-delivered meals
• Assistive technology and personal emergency response systems
• Home modifications (ramps, grab bars, door widening)
• Caregiver respite (temporary relief for the primary caregiver)
• Monitored In-Home Caregiving (MIHC) — which can pay a family member or spouse to serve as the live-in caregiver

The MIHC benefit is particularly significant for dementia families. Under the CCW, a qualifying family member can become the paid, state-funded caregiver, receiving compensation for care they would otherwise provide without support.

The Smart Strategy: Use Both

For most families, the practical approach is not either-or — it's LT-PCS first, then CCW when a slot opens.

If your parent's income is under $994 and they meet the NFLOC standard, enroll them in LT-PCS immediately. There is no wait. At the same time, call Louisiana Options in Long-Term Care at 1-877-456-1146 and register them on the CCW Request for Services Registry. They receive immediate in-home personal care through LT-PCS while their name climbs the waiver waitlist.

When a CCW slot opens, they transition from the limited LT-PCS service package to the full waiver benefit set — gaining support coordination, home modifications, respite, and potentially the MIHC payment option.

One detail that matters: if your parent is approved for 32 or more hours per week of LT-PCS, they qualify for CCW priority Group 5 — individuals who will require nursing home placement unless they receive an expedited waiver slot. This can significantly shorten the wait.

When Neither Program Works

If your parent's income exceeds $994 but falls under $2,982, they do not qualify for LT-PCS but can qualify directly for the CCW — assuming a slot is available. If it isn't, they face the waitlist with no immediate state-funded home care. Private-pay home health aides bridge the gap, but at $25 or more per hour in most Louisiana markets, costs add up fast.

For parents with income above $2,982, the Medically Needy Spend-Down pathway applies to the CCW. Under this program, excess income is offset by medical expenses, including the projected cost of waiver services. The math is specific to each family's situation.
