Louisiana Medicaid Authorized Representative for Medicare Savings Programs
Why You Need Formal Authorization to Help Your Parent
If you're trying to help a parent apply for Louisiana's Medicare Savings Programs, you'll hit a wall the moment you call the LDH Medicaid customer service line or walk into a regional office. Louisiana Medicaid caseworkers enforce HIPAA and state privacy regulations strictly — they generally require formal authorization on file before discussing your parent's case or sharing application status with you.
A general financial power of attorney is not enough on its own. Louisiana's Medicaid system requires a specific state form to be processed before a caseworker will speak with you. Many applications stall because a caregiver submitted everything correctly but can't follow up, check status, or respond to verification requests without this authorization in place.
BHSF Form AR — The State Authorization Form
The primary form is BHSF Form AR (Medicaid Authorized Representative Form), also referenced as BHSF Form 1-A Appendix C in LDH policy. Executing this form gives you legal authority to:
- Sign Medicaid applications on behalf of your parent
- Submit renewal documentation
- Receive copies of notices and correspondence
- Speak directly with LDH caseworkers about the case
- Elect health plan options
The form requires the explicit signature of the applicant — your parent, or someone with existing legal authority such as a curator, guardian, or power-of-attorney holder. You cannot sign as both the applicant and the representative. Commercial entities and Medicaid Application Center staff are also prohibited from signing Form AR on behalf of an applicant.
Submit the completed form with the MSP application (Form 1-MB), or separately by mail to the Medicaid Application Office at P.O. Box 91278, Baton Rouge, LA 70821-9278, or by fax to 1-877-523-2987.
If Your Parent Has Dementia or Can't Sign
When a parent cannot sign Form AR due to cognitive decline, you need legal authority first. In Louisiana, the relevant instruments are:
- Durable power of attorney (mandate): If your parent executed a mandate before losing capacity, the mandatary (agent) can sign Form AR on the principal's behalf. Louisiana uses "mandate" for what many states call a power of attorney.
- Curatorship (guardianship): If no mandate exists and your parent lacks capacity, you'll need a court-appointed curatorship through the district court. This is a legal proceeding that takes time and money.
The practical takeaway: get Form AR signed while your parent is still able to sign it. Even if you're not applying for MSP right now, having the authorized representative designation on file means you can act immediately when the need arises.

Managing Louisiana MSP From Out of State
A significant number of caregivers managing a parent's Medicare costs live in Texas, Georgia, California, or elsewhere outside Louisiana. The authorized representative designation works the same regardless of where you live — it's tied to the parent's Louisiana Medicaid case, not your location.
Once Form AR is processed, you can:
- File the initial MSP application online through MyMedicaid.la.gov or by mail
- Upload verification documents through the LaMEDS Self-Service Portal
- Call 1-888-342-6207 to check application status
- Respond to renewal notices and verification requests remotely
- File a fair hearing request if the application is denied
The portal doesn't provide real-time tracking — case status labels like "Pending Verification" don't specify which document is missing. When something is unclear, you'll need to call the customer service line. Authorization on file means they'll actually talk to you.
Form AR Does Not Work at Social Security
One common mistake: assuming the LDH authorized representative form covers all agencies. It does not. If your parent needs help at the Social Security Administration — for example, to file a conditional Part A application or resolve a buy-in delay — you need separate SSA authorization. Form SSA-1696 appoints a representative; Form SSA-11 is used to apply to serve as a representative payee for benefit payments. The LDH Form AR and SSA forms are not interchangeable.
The Louisiana Medicare Savings Programs & Extra Help Guide includes the step-by-step process for establishing authorized representative status, what to do when a parent can't sign, and how to coordinate between LDH and SSA when managing a parent's Medicare costs remotely.
