South Carolina Medicare Savings Program Authorized Representative: How to Apply for a Parent From Anywhere
Why Remote Caregivers Need Authorized Representative Status
SCDHHS enforces strict privacy rules on every Medicare Savings Program case. Without formal authorization on file, caseworkers cannot discuss your parent's eligibility, share processing updates, or send notices to your address. You can fill out the application for your parent as an informal helper, but you are locked out of every conversation that follows.
That lockout becomes a serious problem when your parent lives in a rural South Carolina county — Barnwell, Williamsburg, Bamberg — and you are managing their care from Charlotte, Atlanta, or further. The 45-day processing window ticks whether or not anyone picks up the phone.
How to Establish Authorized Representative Status
South Carolina uses SCDHHS Form 1282 ("Authorization for Release of Information and Appointment of Authorized Representative") to create the legal link between you and your parent's case.
Both the applicant and the representative must sign Form 1282. If your parent has the capacity to sign, the process is straightforward — print the form, have them sign their section, sign yours, and submit it alongside the main application (Form 3400).
If your parent lacks capacity to sign, you need to attach proof of legal authority. A durable power of attorney, guardianship order, or conservatorship order satisfies SCDHHS. Without one of these documents, the agency will not accept the representative designation.
Once Form 1282 is processed, SCDHHS treats you as the applicant's authorized voice. You receive all official correspondence, can sign renewal paperwork, and speak directly with eligibility staff about income calculations, missing documents, and processing delays.
Filing the Application From Out of State
You do not need to visit a county office in person. SCDHHS accepts applications through five channels, and four work from anywhere:
Fax to (888) 820-1204. Fax the completed Form 3400, Form 1282, and all supporting documents (Medicare card copy, Social Security award letter, bank statements, burial fund documentation) in a single transmission. Keep the confirmation page.
Email the packet as a PDF attachment to [email protected]. This routes directly into the central imaging system alongside faxed submissions.
Mail to SCDHHS Central Mail, P.O. Box 100101, Columbia, SC 29202-3101. Use certified mail with return receipt if you want proof of delivery.
The online portal at apply.scdhhs.gov works too, though uploading supporting documents through the portal can be unreliable — the phone number input field has been reported to malfunction, and the document upload occasionally times out.

Documents You Should Gather Before Filing
Having everything ready prevents the most common delay: an Information Request Notice that pauses the processing clock while SCDHHS waits for missing paperwork.
Assemble these before submitting:
- Copy of your parent's Medicare card (red, white, and blue card)
- Most recent Social Security award letter showing gross monthly benefit
- Bank statements for the month of application and prior months (checking, savings, CDs)
- Any pension or VA benefit letters
- Life insurance policy face-value summaries (if total face value exceeds $1,500)
- Proof of funds set aside for burial in a separate, designated account (if claiming the $1,500 per-person burial-fund exclusion)
- Pre-paid burial contract documentation showing the contract is irrevocable and non-refundable (if claiming the exclusion for a pre-need contract)
- Your signed Form 1282 with proof of legal authority if your parent cannot sign
What Happens After You Submit
SCDHHS has 45 calendar days to process a Medicare Savings Program application. In practice, most applications take 30 to 45 days when the documentation is complete.
As the authorized representative, you will receive the approval or denial notice at your address. If approved for QMB, coverage begins the first day of the month after the approval date. SLMB and QI approvals can be backdated up to three months if your parent qualified during those prior months.
Once the buy-in record reaches Social Security, the $202.90 monthly Part B premium deduction stops — though that data exchange between SCDHHS, CMS, and SSA typically takes an additional 60 to 90 days. SSA processes a lump-sum reimbursement for all premiums deducted after the coverage effective date.
Staying in the Loop During Processing
Call the Healthy Connections help line at 1-888-549-0820 to check application status. With Form 1282 on file, the agent can pull up the case and tell you whether documents are missing, whether the case has been assigned, and what the expected timeline looks like.
If you need language interpretation services, request free translation through the same number (TTY: 1-888-842-3620).
The South Carolina Medicare Savings Programs & Extra Help Guide walks through the entire remote filing process step by step, including a pre-filled document checklist and the exact Form 3400 sections that apply to MSP-only applications — so you skip the long-term care pages that do not apply.
