How to Apply for Maine Medicare Savings Program
There's No Separate MSP Form — Use the General MaineCare Application
Maine doesn't have a standalone Medicare Savings Program application. You file using the same integrated health coverage application used for all MaineCare programs. When applying only for MSP (QMB or QI), fill out the demographic and income sections and leave the asset sections blank — Maine eliminated the asset test for all MSP categories under 22 M.R.S. § 3174-LLL, effective January 1, 2024.
Filing an MSP application also doubles as a DEL (Low Cost Drugs for the Elderly or Disabled) application. If you qualify for MSP and meet DEL's income threshold, enrollment in DEL is automatic — no second form needed.
Four Ways to File
Online through My Maine Connection The state's benefits portal at mymaineconnection.maine.gov lets you create a client account or submit as a guest. You can upload verification documents directly. The portal occasionally experiences timeouts, particularly during peak enrollment periods — if it stalls, the phone and mail routes work just as well.
By phone Call the OFI Eligibility Telephonic Center at 1-855-797-4357. An eligibility specialist captures your responses, generates the electronic application, and accepts a telephonic signature. This is the fastest route for adult children applying as Authorized Representatives from out of state.
By mail or fax Send the completed paper application to the centralized processing center:
- Mail: Office for Family Independence, 114 Corn Shop Lane, Farmington, ME 04938
- Fax: (207) 778-8429
- Email: [email protected]
In person Walk into any regional DHHS district office. Staff can help complete the application on site.
Documents You'll Need
OFI runs automated data checks through the Social Security State Verification Exchange System (SVES) and BENDEX to verify income and Medicare entitlement. You may not need to provide much paperwork if their electronic match succeeds. Have these ready in case manual verification is required:
- Medicare card (showing the Medicare Beneficiary Identifier)
- Most recent Social Security award letter or annual COLA notice
- Private pension or VA benefit statements (if applicable)
- Proof of Maine residency (utility bill, lease, or Maine driver's license)

Filing as an Authorized Representative
An adult child can manage the entire application on behalf of a parent. To set this up:
- Complete the Authorized Representative designation section within the My Maine Connection portal, or submit a signed written consent form
- Once designated, you have authority to speak with OFI staff, submit documents, track application status, and receive all official notices
This is especially practical for out-of-state adult children managing a parent's care remotely. The designation stays in effect until the beneficiary revokes it.
What Happens After You File
OFI has 45 calendar days to process a standard application (90 days if a disability determination is needed). If they need additional documents, they'll send a notice with a 10-to-14-day response deadline.
Once approved, coverage starts on the first day of the month after the approval decision (QMB) or can be retroactive up to three months before the application (QI). Either way, Social Security takes another 30 to 90 days to stop deducting the Part B premium — and then issues a lump-sum refund for the months since coverage started.
The Maine Medicare Savings Programs guide includes a document checklist, an application tracking log, and step-by-step instructions for each submission route — plus what to do if OFI asks for information you don't have.
