Nobody has arranged home health visits. You need to stop this discharge — and Rhode Island gives you a legal mechanism to do exactly that.

The Fast Appeal Process: Step by Step

Medicare patients in Rhode Island can file a "fast appeal" that legally freezes a hospital discharge while an independent medical reviewer evaluates whether the discharge is clinically appropriate.

Step 1: Call Acentra Health immediately. Acentra Health is Rhode Island's Beneficiary and Family Centered Care Quality Improvement Organization (BFCC-QIO). Their helpline number is 1-888-319-8452. You can also fax documentation to 1-844-878-7921.

Step 2: File before midnight on the scheduled discharge day. The deadline is firm: your appeal must be filed no later than the actual day of the scheduled discharge. If the hospital says your parent is leaving Thursday, you must contact Acentra Health by Thursday. Filing even one day late forfeits this expedited review pathway.

Step 3: Gather your documentation. Have these ready when you call:

  • Your parent's Medicare Beneficiary Identifier (MBI) — printed on their red, white, and blue Medicare card
  • The name and phone number of the attending physician
  • The specific clinical reasons you believe the discharge is unsafe (inability to perform ADLs, unresolved medical issues, inadequate home support)

What Happens After You File

Once Acentra Health receives your appeal, three things happen simultaneously:

  1. The discharge freezes. The hospital cannot send your parent home while the review is pending.
  2. The hospital must respond. By noon the day after Acentra notifies them of the appeal, the hospital must provide a Detailed Notice of Discharge (Form CMS-10066) explaining the clinical justification for the discharge.
  3. No bill accrues. The hospital cannot charge your parent for the continued stay during the QIO review period.

Acentra Health typically issues a decision within one business day for fast appeals.

If the Appeal Is Denied

If Acentra Health upholds the discharge, you still have options:

  • Fast Reconsideration: Request a second-level review by noon of the calendar day following Acentra's notification. This goes to the Qualified Independent Contractor (C2C Innovative Solutions for Part A East), a completely separate entity from Acentra.
  • Stay at your own cost: If your parent remains in the hospital past noon of the day following the initial QIO denial without filing a reconsideration, they assume full financial liability for the bed.
  • Administrative Law Judge hearing: A third-level appeal can be requested within 60 days if the reconsideration is also denied.

Do Not Wait Until Discharge Morning

The biggest mistake families make is waiting until the discharge planner hands over paperwork on the morning of discharge. By then, the bed has been assigned to the next patient, transportation has been arranged, and the institutional momentum is overwhelming.
