HoosierRx: Indiana's Prescription Drug Premium Assistance Program

What HoosierRx Does

HoosierRx is Indiana's State Pharmaceutical Assistance Program (SPAP), run by FSSA's Office of Medicaid Policy and Planning. It pays up to $70 per month toward your Medicare Part D or Medicare Advantage prescription drug plan premium. That's up to $840 a year in relief.

The payment goes directly from the state to the insurance carrier — you never see a check. And HoosierRx covers only the monthly premium. It does not pay for deductibles, copayments, or coinsurance at the pharmacy counter.

Who Qualifies

HoosierRx has specific eligibility rules that differ from the Medicare Savings Programs:

  • Age 65 or older — disabled individuals under 65 are excluded, even if they have Medicare
  • Indiana resident
  • Annual income of $23,940 or less (single) or $32,460 or less (couple) in 2026
  • No asset test — HoosierRx does not count your bank balance, home, or other resources
  • Must have a Part D plan enrolled with a carrier that has an active SPAP contract with HoosierRx

The eligibility wrinkle that trips people up: if you receive full Extra Help (the federal Low-Income Subsidy), you cannot get HoosierRx. Full LIS already covers your Part D premium. HoosierRx exists for seniors who were denied Extra Help (typically because their assets exceeded the federal LIS limit of $18,090 single / $36,100 couple) or who receive only partial Extra Help.

HoosierRx vs. Extra Help: Understanding the Boundary

This is the most confusing intersection in Indiana's Medicare assistance landscape. Here's how the two programs relate:

Extra Help is a federal program through Social Security that reduces Part D premiums, deductibles, and drug copayments. Under the Inflation Reduction Act, all Extra Help enrollees now receive full benefits — $0 premium for benchmark plans, $0 deductible, and copays of $5.10 (generic) or $12.65 (brand-name).

HoosierRx is a state program that only covers the Part D premium, up to $70/month.

If you qualify for Extra Help, your Part D premium is already $0 on benchmark plans — HoosierRx would have nothing to pay. That's why full LIS recipients are excluded.

HoosierRx fills a gap for seniors whose assets push them over the federal Extra Help limit but whose income stays at or below Indiana's $23,940 single-person threshold. If SSA denied your Extra Help application because of assets, HoosierRx may be your route to premium relief.
