Help Paying Medicare Premiums in Iowa

The standard Medicare Part B premium in 2026 is $202.90 per month — $2,434.80 per year, deducted straight from the Social Security check before it hits the bank account. For an Iowa senior living on $1,400 a month, that's nearly 15% of gross income going to one premium.

Iowa's Medicare Savings Programs exist specifically to cover that cost.

Three Programs That Pay the Part B Premium

All three active MSP tiers cover the monthly Part B premium. Which one applies depends on income:

QMB — for individuals with net countable income at or below $1,350/month (couple: $1,824). Covers Part B premium plus all deductibles, coinsurance, and copays. Also provides balance billing protection.

SLMB — for income between $1,350 and $1,616/month single ($1,824–$2,184 couple). Covers the Part B premium only.

E-SLMB/QI — for income between $1,616 and $1,816/month single ($2,184–$2,455 couple). Also covers Part B premium only. Funded from a capped federal allocation, processed first-come, first-served.

All three tiers use net countable income, not gross. Iowa HHS subtracts a $20 monthly disregard from unearned income before comparing against the limits. That's why someone receiving $1,365 in gross Social Security — seemingly above the published QMB cap — can still qualify.

Asset limits are the same across tiers: $9,950 individual, $14,910 couple. The primary home, one vehicle, and burial funds are excluded.

How to Apply

Apply online at hhsservices.iowa.gov, by phone at 1-855-889-7985, on paper using Form 470-5170, or in person at a county office. You don't choose a specific tier. Iowa HHS evaluates income and assets and assigns the right program.

Processing takes 45 calendar days, or 90 if a formal disability determination is required. Have the most recent Social Security Award Letter, bank statements from the past month, and proof of Iowa residency ready.

What Happens to the Social Security Deduction

After Iowa HHS approves MSP coverage, it transmits a buy-in record to CMS, which tells Social Security to stop withholding the Part B premium from the monthly check. This adjustment typically takes 30 to 60 days to show up in the direct deposit.

During that lag, Social Security continues deducting the premium as usual. Every dollar withheld after the MSP effective date gets refunded — either through the existing direct deposit or a paper check. The refund comes from SSA, not from Iowa HHS.

For SLMB and E-SLMB, the effective date can reach back up to three months before the application month, so eligible premiums withheld during those retroactive months are also refunded.

If premium withholdings haven't stopped within 90 days of the approval notice, contact Iowa HHS buy-in staff or call Social Security at 1-800-772-1213 to request a manual accretion audit. Processing delays beyond 90 days are uncommon but do happen when data transmission cycles fall out of sync.

The Part B Premium Is Automatic — But It's Worth Checking

Most Medicare beneficiaries never think about the Part B premium because it's already deducted before they see their Social Security payment. Pull up a recent Social Security benefit statement (available at ssa.gov or on Form SSA-1099) and look at the Medicare deduction line. If it shows $202.90 or more, and the beneficiary's net countable income falls within the ranges above, they're paying a premium that Iowa HHS would cover.
