Help Paying Medicare Premiums in Idaho

$202.90 a Month Is a Lot on a Fixed Income

The 2026 Medicare Part B premium is $202.90 per month — $2,434.80 a year deducted from Social Security before your parent sees a dollar. For a senior living on $1,400 a month, that's nearly 15% of their income going to a single insurance premium.

Idaho's Medicare Savings Programs exist specifically to pay that premium. Depending on income level, the state can cover Part B alone or Part B plus the Part A premium, hospital deductibles, and all Medicare coinsurance.

Three Programs That Pay the Part B Premium

QMB (Qualified Medicare Beneficiary) covers the Part B premium plus all other Medicare cost-sharing — deductibles, coinsurance, copays. Income limit for 2026: $1,350/month for singles, $1,824 for couples. Resources under $9,950 single, $14,910 couple.

SLMB (Specified Low-Income Medicare Beneficiary) covers the Part B premium. Income limit: $1,616/month single, $2,184 couple. Same resource limits as QMB.

QI (Qualifying Individual) also covers the Part B premium. Income limit: $1,816/month single, $2,455 couple. Same resource limits.

All three programs are administered by the Idaho Department of Health and Welfare. Your parent doesn't need to know which tier they qualify for — DHW's caseworker evaluates the application and places them in the appropriate program.

Part A Premium Help

Most people 65 and older get Part A for free because they or a spouse paid Medicare taxes for at least 40 quarters. But seniors who didn't accumulate enough work credits face a Part A premium of up to $565 per month in 2026.

QMB covers that Part A premium too. And because Idaho is a Part A Buy-In State, a senior who lacks 40 quarters can do a conditional Part A enrollment at Social Security any time of year — not just during the January–March General Enrollment Period. SSA won't send a premium invoice as long as the conditional enrollment is tied to a QMB application with DHW. Late enrollment penalties are waived.

How the Premium Reimbursement Works

Once DHW approves the application, it sends an electronic file to CMS and SSA. For SLMB and QI enrollees, the Part B deduction from Social Security stops within 30 to 90 days. SSA then mails a retroactive reimbursement check covering premiums deducted during the processing period.

The lag matters. Your parent will continue seeing the $202.90 deduction from their Social Security check for one to three months after approval. This isn't a sign that the application was denied — it's the normal timeline for SSA to update its payroll system.

SLMB and QI coverage can be backdated up to three calendar months before the month of application, provided the applicant met the criteria in those months, so the reimbursement check can be substantial. QMB is prospective only — coverage starts the first of the month after the approval decision.

Bonus: Automatic Extra Help for Prescriptions

Every MSP tier — QMB, SLMB, and QI — automatically triggers Extra Help (Low-Income Subsidy) for Part D prescription drugs. This eliminates the Part D plan premium on benchmark plans and caps copays at $5.10 for generics and $12.65 for brand-name drugs. The 2026 Part D out-of-pocket cap is $2,100 for standard enrollees, but Extra Help recipients pay far less.

Where to Start

Apply through idalink.idaho.gov, by mailing Form HW2019, by fax to 1-866-434-8278, or by calling DHW at 1-877-456-1233. Free counseling is available through SHIBA at 1-800-247-4422.

The Idaho Medicare Savings Programs & Extra Help Guide includes a side-by-side comparison of all three tiers, an eligibility worksheet that walks through the income disregard calculation, and a document checklist built around what DHW actually requires for a first-time application.
