Hawaii Social Security Check Reduced for Medicare Part B: What Happened

Why the Check Suddenly Dropped

When a senior's monthly Social Security deposit shrinks by exactly $202.90, the cause is almost always a Medicare Part B premium deduction that was not being withheld before. In Hawaii, this happens most often when a Medicare Savings Program authorization — QMB, SLMB, or QI — lapses or is terminated by Med-QUEST, and SSA resumes deducting the Part B premium from the benefit check.

The deduction appears automatically. SSA does not call or warn before taking it. The senior sees a smaller direct deposit and may not connect it to their Med-QUEST status.

In September 2025, Hawaii families reported receiving SSA letters saying QI no longer covered their Part B premiums. A family member said Med-QUEST staff told them the notices were sent in error and the QI authorizations remained active. If a Part B deduction appears, confirm the current authorization with Med-QUEST.

Common Causes in Hawaii

Renewal lapse. Med-QUEST sends renewal forms (Form N-14 or DHS 1100B-2) with a 30-day response deadline. If the form is not returned in time — common when mail is slow to neighbor islands or when an adult child on the mainland manages paperwork — Med-QUEST closes the MSP case. SSA's system picks up the closure and resumes the Part B deduction.

System mismatch. The state buy-in file exchange between Med-QUEST and CMS is a batch process, not real-time. Timing errors can cause SSA to resume deductions before Med-QUEST has corrected a case on its end.

Income increase at recertification. If a cost-of-living adjustment (COLA) to Social Security raises countable income above the current MSP tier limit, Med-QUEST may move the enrollee from QMB to SLMB, or from SLMB to QI. SLMB and QI also pay the Part B premium for eligible enrollees.

How to Fix It

Call Med-QUEST customer service at 1-800-316-8005 (or your island's local eligibility office) and confirm whether the MSP authorization is still active. If it was terminated due to a missed renewal, submit the renewal immediately — SLMB and QI allow up to three months of retroactive coverage if the enrollee was eligible during those months, so premiums deducted in that period can be refunded.

If the MSP case is active and SSA deducted the premium in error, Med-QUEST needs to retransmit the buy-in file to CMS. Ask for the date of the most recent buy-in transmission and a case reference number. It typically takes 60 to 90 days for SSA to update its payment system and stop the deduction, but the refund for over-deducted months arrives as a lump sum once the system catches up.

Do not ignore the deduction and assume it will self-correct. Every month the premium comes out of the check is a month where your parent is effectively paying $202.90 they should not owe.

The Social Security Fairness Act and Hawaii Beneficiaries

The Social Security Fairness Act (signed January 5, 2025) repealed the Government Pension Offset (GPO) and Windfall Elimination Provision (WEP). For Hawaii retirees who worked in government jobs — state, county, or federal positions not covered by Social Security — this means their Social Security spousal or survivor benefits are no longer reduced or eliminated by those provisions.

The practical effect for MSP eligibility: some beneficiaries now receive higher Social Security payments than before the repeal, which could push their gross income above a lower MSP tier (like QMB) into a higher one (like SLMB). Others who never applied for Social Security spousal benefits because GPO would have zeroed them out should now file a new claim with SSA — the adjustment is not automatic for people who never applied.

If your parent's check increased due to the Fairness Act, recheck their MSP eligibility tier against Hawaii's 2026 income limits. They may still qualify for SLMB or QI even if QMB is now out of reach.
