DHS 1169 Evaluation for Placement of Liens — documents property ownership status
DHS 1169A Maintenance of Lien Status — maintains the lien-status record
DHS 8003 Annuity Reporting Form — required if the parent owns any annuity contracts
DHS 8004 Home Equity Survey — submitted with the long-term-care application and used for annual home-equity review

DHS 1100 and 1100B can be submitted online through medical.mybenefits.hawaii.gov or in person at your local Med-QUEST eligibility office (offices on Oahu, Maui, Kauai, and Hawaii Island in both Hilo and Kona).

Why They Need 60 Months of Bank Statements

The 60-month bank statement requirement exists because of the lookback period. Med-QUEST reviews five years of financial activity to identify any transfers made for less than fair market value — gifts to children, property transfers, payments to family members, or any transaction that reduced the parent's assets without receiving equivalent value in return.

The eligibility worker will review:

  • Checking account statements — all accounts, every month, for 60 months prior to the application date
  • Savings account statements — same 60-month window
  • Investment and brokerage statements — quarterly or monthly, depending on the institution
  • Retirement account statements (IRA, 401(k)) — annual or quarterly statements covering the full period
  • Life insurance policy statements — showing current cash values and any policy loans
  • Real estate records — deeds, transfers, sales within the lookback period
  • Vehicle title history — any transfers of vehicle ownership

If your parent closed a bank account three years ago, you still need the statements from before the closure. Contact the bank — most institutions can retrieve archived statements for a fee. Credit unions and online banks typically have digital records going back further than five years.

Large or unusual transactions get flagged. Expect the eligibility worker to ask for explanations of transactions that do not have an obvious purpose (paying a bill, medical expense, home repair). Keeping receipts and invoices alongside the bank statements speeds up the verification process significantly.

Beyond the forms and bank statements, prepare:

  • Social Security award letter (current year)
  • Pension statements showing monthly payment amounts
  • Medicare card and any supplemental insurance cards
  • Property tax assessment or deed for the primary home
  • Vehicle registration and title
  • Burial plan contracts (if prepaid)
  • Health insurance premium statements (Medicare Part B, Medigap)
  • Proof of Hawaii residency (utility bill, lease, property tax bill)
  • Power of Attorney documentation if filing on behalf of the parent

How Long Processing Takes

Med-QUEST has a legal obligation to process standard Medicaid applications within 45 days. However, complex long-term care applications involving asset verification frequently take 60 to 90 days.

The most common delay: missing or incomplete bank statements. The eligibility worker sends a request for additional information, the family takes weeks to track down old records, and the practical timeline can extend. Front-loading all 60 months of statements with the initial application packet is the single most effective way to reduce processing time.
