Hawaii Extra Help Medicare: Part D Prescription Drug Savings

What Extra Help Does for Prescription Costs

Extra Help — formally called the Part D Low-Income Subsidy — is a federal program that slashes what Medicare beneficiaries pay for prescription drugs. In 2026, Extra Help caps copays at $5.10 for generics and $12.65 for brand-name medications. It also eliminates the Part D deductible entirely and closes the coverage gap.

For a senior taking three or four prescriptions monthly, the savings can run several hundred dollars a month, especially for brand-name drugs that would otherwise cost $50 to $100+ per fill under standard Part D.

Two Paths to Extra Help in Hawaii

Path 1: Automatic through Medicare Savings Programs. If your parent enrolls in QMB, SLMB, or QI through Hawaii's Med-QUEST Division, they automatically qualify for Extra Help. No separate application. CMS handles the enrollment once the MSP goes through.

This is the path most Hawaii seniors should take first, because MSP enrollment also pays the Part B premium ($202.90/month) and — in the case of QMB — deductibles and copays on all Medicare-covered services.

Path 2: Direct application through SSA. Seniors who earn too much for any MSP tier but still have limited income can apply for Extra Help directly through Social Security. Standalone Extra Help uses separate income and resource limits from the MSPs; the 2026 Hawaii amounts are listed below.

The SSA application is at ssa.gov/extrahelp or by calling 1-800-772-1213. In 2026, Hawaii's standalone Extra Help income limits are less than $27,540 per year (about $2,295/month) for an individual and $37,335 per year (about $3,111/month) for a married couple, based on countable income. The countable resource limits are $16,590 for an individual and $33,100 for a married couple, before any applicable burial-fund exclusion. SSA also sends an automatic referral to Med-QUEST when someone applies for Extra Help, which can trigger an MSP screening.

The Automatic MSP-to-Extra-Help Link

When Med-QUEST approves a Medicare Savings Program application, the approval record flows to CMS, which flags the beneficiary for automatic Extra Help enrollment. The beneficiary doesn't fill out a second form and doesn't need to contact SSA.

The practical effect: one Med-QUEST application — Form DHS 1100 plus DHS 1100B — can unlock both premium relief and prescription drug savings in a single administrative step.

Extra Help enrollment can also happen in reverse. When someone applies for Extra Help through SSA and gets approved, SSA sends an electronic referral (called a MIPPA file) to Med-QUEST. MQD then sends the applicant a notice asking them to complete the supplemental DHS 1100B form for MSP screening.

What Extra Help Doesn't Cover

Extra Help reduces Part D prescription costs but does not affect Medicare Part A or Part B charges. It doesn't cover hospital deductibles, doctor visit copays, or Part B premiums. For those, your parent needs QMB (which covers all of the above) or SLMB/QI (which cover the Part B premium).

Extra Help also doesn't help with drugs not on the Part D formulary or prescriptions filled at non-network pharmacies. Part D plan choice still matters — Hawaii SHIP (808-586-7299 on Oahu, 1-888-875-9229 from neighbor islands) provides free counseling on picking the right Part D plan.

Hawaii Rx Plus Is Not a Substitute

Families sometimes search for the Hawaii Rx or Hawaii Rx Plus program. Those state-level prescription discount initiatives were discontinued after Medicare Part D launched. The "Hawaii Rx Card" that shows up in searches is a private commercial discount card — not a government program — and it cannot be combined with Part D coverage at the pharmacy counter.

Extra Help through Medicare Part D is the real prescription cost relief for low-income seniors in Hawaii. The fastest way to get it is through an MSP application.
