What the GUIDE Model Is

The GUIDE Model — Guiding an Improved Dementia Experience — is a federal demonstration program run by the Centers for Medicare & Medicaid Services (CMS). It launched in July 2024 with participating health systems across the country, including providers serving Missouri patients.

The core idea: Medicare beneficiaries with a dementia diagnosis get assigned a care navigator who coordinates their medical care, connects the family with community resources, and provides a structured care plan. The model also includes up to $2,500 annually in respite care benefits for unpaid caregivers — one of the only Medicare-funded respite benefits available anywhere.

This is not a state program. It operates through Medicare, so eligibility is based on Medicare enrollment rather than MO HealthNet or Missouri-specific income limits.

How It Works for Missouri Families

CMS eligibility criteria include a qualifying dementia diagnosis or clinician attestation, Medicare Parts A and B as the primary coverage, and residence in a setting eligible under the model. People enrolled in Medicare Advantage or PACE, or living long-term in a nursing facility or memory-care unit, generally do not meet GUIDE eligibility. A participating GUIDE care partner must also be able to serve the person's area.

Once enrolled, the patient and their primary caregiver work with a dedicated care navigator — typically a social worker or nurse — who develops a comprehensive care plan. The navigator helps coordinate between specialists, primary care physicians, home health agencies, and community organizations like Area Agencies on Aging.

The respite benefit is the piece that gets the most attention from families. For an eligible participant, the GUIDE care partner can arrange up to $2,500 per year in respite services, such as adult day programs, in-home respite, or facility-based respite in an eligible setting. This is separate from respite hours available through Missouri's Aged and Disabled Waiver or the Structured Family Caregiving Waiver, and it is not an unrestricted cash payment.

Limitations to Understand

The GUIDE Model is a demonstration — it runs for a fixed period and may not be renewed. Families should not build long-term care plans around it as a permanent benefit.
