IRIS: The Primary Path to Paid Family Caregiving
Wisconsin's IRIS (Include, Respect, I Self-Direct) program is a Medicaid Home and Community-Based Services waiver that gives participants direct control over a monthly care budget. Unlike Family Care's managed-care model, IRIS lets your parent hire their own workers — including adult children, siblings, and in many cases, spouses.
The process works like this: your parent enrolls in IRIS through the county Aging and Disability Resource Center (ADRC). An IRIS Consultant Agency helps develop a support plan that identifies the care tasks needed and the hours required. A Fiscal Employer Agent (FEA) — companies like GT Independence or PPL — handles payroll processing, tax withholding, and workers' compensation insurance.
Your parent becomes the employer. You become the employee. The FEA pays you directly, withholds taxes, and files the required employment reports.
The critical rule: if you're the person being paid to provide care, you cannot simultaneously serve as your parent's IRIS Representative (the person who manages the budget and makes program decisions). Someone else — another family member, a trusted friend — must fill that role. This separation prevents conflicts of interest and is strictly enforced.
What IRIS Pays Family Caregivers
IRIS caregiver pay rates in Wisconsin typically range from $13 to $20 per hour, depending on the county, the complexity of care tasks, and the participant's approved budget. Rates for Supportive Home Care (non-medical assistance with ADLs like bathing, dressing, and toileting) and Self-Directed Personal Care fall within this range.
Your parent's total IRIS budget is calculated based on their Long-Term Care Functional Screen results and the services identified in their support plan. The budget covers not just your wages but also employer-side payroll taxes, workers' compensation, and the FEA's administrative fees.
Family Care: More Limited, but Possible
Under Wisconsin's Family Care managed-care program, paying family members is more restricted. The Managed Care Organization (MCO) builds the care plan and selects providers. In some cases, the MCO may approve a family member as a paid personal care worker, but this is at the MCO's discretion and subject to their provider credentialing requirements.

Veteran Programs That Pay Family Caregivers
If your parent is a veteran, two additional programs may apply:
VA Aid and Attendance provides a monthly pension supplement (up to $2,424 per month for a single veteran under the rates effective December 1, 2025) that can be used to pay a family caregiver. The veteran must need help with at least two ADLs or be housebound.
The VA Program of Comprehensive Assistance for Family Caregivers provides a monthly stipend directly to the caregiver of a veteran with a serious service-connected injury. Stipend amounts are based on the local market rate for professional home care.
How to Get Started
- Contact your county ADRC and request the Long-Term Care Functional Screen (LTCFS). Your parent must meet Nursing Facility Level of Care to qualify for IRIS or Family Care.
- Choose IRIS when offered the program selection. Tell the ADRC enrollment specialist that self-directed care with family caregiving is your goal.
- Designate a non-caregiver IRIS Representative — this person manages the budget and signs off on the support plan. It cannot be the same person providing paid care.
- Select a Fiscal Employer Agent (GT Independence or PPL are common in Wisconsin) to handle payroll and employment compliance.
- Complete the employment paperwork — background check, I-9, W-4, and the FEA's onboarding forms.
