Fall Prevention and Home Safety for North Dakota Seniors

Falls are the leading cause of injury-related hospitalization for seniors in North Dakota. One fall can trigger a cascade: emergency surgery, rehabilitation, functional decline, and a forced move to a nursing facility. Most falls happen at home, and most are preventable with modifications that cost far less than a single ER visit.

The Home Safety Audit

A structured room-by-room evaluation identifies the specific hazards in your parent's home before they cause an injury:

Bathroom — the highest-risk room. Check for grab bars next to the toilet and inside the shower or tub, non-slip mats or textured strips on wet surfaces, a shower chair or transfer bench, and adequate lighting. Tub-to-shower conversions eliminate the dangerous step-over that causes the most bathroom falls.

Hallways and stairs — look for loose carpet edges, poor lighting, missing handrails, and clutter. Every stairway should have secure handrails on both sides. Nightlights or motion-activated lighting along the path from bedroom to bathroom prevents nighttime falls.

Kitchen — heavy items stored at waist height (not on high shelves requiring reaching), non-slip flooring, accessible counter seating for tasks that cause standing fatigue.

Entrances — wheelchair ramps or handrailed steps, weather-protected surfaces to prevent ice accumulation, doormats that lie flat without curling edges.

In North Dakota, winter creates hazards that families in milder climates never face. Unplowed walkways, ice on steps, and frozen door locks can strand a parent inside or cause falls during the most dangerous season.

State-Funded Modifications

North Dakota covers home modifications through the Medicaid HCBS Waiver and SPED as environmental modifications within the care plan. Grab bars, ramps, lighting upgrades, non-slip surfaces, and bathroom conversions are all eligible expenses.

The case manager authorizes modifications after a home safety evaluation, and the state pays licensed contractors directly. Families enrolled in SPED (assets under $50,000) pay on a sliding fee scale. Families on the HCBS Waiver pay nothing beyond their standard monthly client share.

Personal Emergency Response Systems (PERS) — wearable devices with emergency call buttons — are covered under SPED, Ex-SPED, and the HCBS Waiver. For parents who live alone and are at high risk of falls, PERS provides 24/7 monitoring and one-button emergency dispatch.

VA Aid and Attendance for Veterans

If your parent is a wartime veteran or the surviving spouse of one, the VA Aid and Attendance benefit provides a monthly pension supplement to cover home care costs. The benefit is income-based and does not require a service-connected disability.

VA Aid and Attendance is separate from North Dakota's Medicaid and SPED programs and can be combined with them. The additional monthly income can fund private-duty home care hours, home modifications, or adaptive equipment that exceeds state program limits.

Application goes through the VA regional office or a Veterans Service Organization. Processing times vary, but the benefit is retroactive to the application date once approved.

Building Prevention Into the Care Plan

Do not treat fall prevention as a separate project. When your parent enrolls in any North Dakota home care program, request that the initial assessment include a formal home safety evaluation.
