Extra Help for Medicare in Montana: How to Apply and What It Covers

What Extra Help Does

Extra Help — officially the Medicare Part D Low-Income Subsidy — slashes your prescription drug costs. If you qualify, it eliminates your Part D deductible, wipes out the coverage gap entirely, and caps your copayments. For those with full Medicaid or QMB status, 2026 copays drop to $1.60 for generics and $4.90 for brand-name drugs. For others who qualify directly through SSA, caps are $5.10 and $12.65.

In 2026, Part D out-of-pocket costs are capped at $2,100. Extra Help turns prescription costs from a monthly worry into a manageable line item.

Two Ways Montana Residents Get Extra Help

Automatically through MSP approval. If Montana DPHHS approves you for QMB, SLMB, or QI, you're deemed eligible for Extra Help — no separate application needed. DPHHS sends daily eligibility feeds to CMS, and you'll get an automatic notice (the purple letter) confirming your LIS status.

Directly through Social Security. If your income is at or below 150% of the federal poverty level ($1,816/month for an individual, $2,450/month for a couple) and your countable resources are within the $18,090 (individual) or $36,100 (couple) limits, you can apply for Extra Help at ssa.gov/medicare/part-d-extra-help or by calling Social Security at 1-800-772-1213.

The SSA route matters even if you're also applying for an MSP, because filing for Extra Help triggers a data transfer to Montana DPHHS under the MIPPA rules. DPHHS treats this incoming data as an initiated MSP application and uses your SSA filing date as your protective filing date for state benefits.

How to Apply

For direct SSA Extra Help, you can:

  • Apply online at ssa.gov/medicare/part-d-extra-help
  • Call 1-800-772-1213 (TTY: 1-800-325-0778)
  • Visit your local Social Security office

The SSA application is simpler than the state MSP application — fewer asset categories, no face-to-face requirement. Keep the decision notice, since Big Sky Rx may require a copy.

If you're applying for Big Sky Rx and your income is at or below 150% FPL, you'll need the SSA decision letter — approval or denial — before the Senior and Long Term Care Division will process your Big Sky Rx application. File for Extra Help first, then submit the determination letter with your Big Sky Rx application.

What Happens After Approval

CMS assigns you to a benchmark Part D plan in Montana if you haven't chosen one yourself. As an LIS beneficiary, you get a Special Enrollment Period to switch Part D plans once per calendar quarter during the first nine months of the year — far more flexibility than the standard annual open enrollment.

For help choosing the right plan, contact Montana SHIP at 1-800-551-3191. SHIP counselors compare plans for free and have no financial incentive to steer you toward any particular option.

The MSP + Extra Help + Big Sky Rx Stack

These three programs layer on top of each other. MSP covers your Part B premium (and more under QMB). Extra Help covers Part D copays and the deductible. Big Sky Rx covers up to $50.60/month of Part D premium that Extra Help doesn't reach.

The key is filing in the right order: apply for Extra Help and MSP simultaneously, get both decision letters, then file Big Sky Rx with the Extra Help determination attached.

The Montana Medicare Savings Programs guide walks through the three-layer filing sequence with timelines and document checklists for each agency.
