Yes — but not the way most families expect. Washington's Medicaid program (Apple Health) doesn't simply write a check to any memory care facility your parent walks into. Coverage flows through specific programs with specific eligibility requirements, and knowing which program applies to your parent's situation is the difference between $8,229/month out of pocket and state-funded care.

The Specialized Dementia Care Program (SDCP)

SDCP is Washington's primary Medicaid-funded pathway for dementia residents in Assisted Living Facilities. It provides enhanced daily reimbursement rates to ALFs that hold Expanded Adult Residential Care Specialized Dementia Care (EARC-SDC) contracts with DSHS.

To qualify, your parent must meet all four criteria:

  1. Financial eligibility for Apple Health (income ≤ $2,982/month or qualifies through medically needy spend-down; countable assets ≤ $2,000)
  2. Functional eligibility for Community First Choice — meeting Nursing Facility Level of Care on the CARE assessment
  3. Cognitive Performance Scale score of 3 or higher on the CARE assessment, representing moderate-to-severe cognitive impairment
  4. At least one active challenging behavior (wandering, exit-seeking, combativeness) requiring specialized intervention, OR requiring direct assistance with eating
  5. Documented irreversible dementia diagnosis from a licensed healthcare practitioner

Not every ALF with a "memory care" sign on the door holds an SDCP contract. You need to confirm the facility is DSHS-contracted before assuming Medicaid will cover the stay.

Other Medicaid Pathways for Dementia Care

SDCP covers memory care in ALFs, but Medicaid also funds dementia care through other channels:

Nursing Home Medicaid

Full Medicaid coverage in skilled nursing facilities — the highest level of care. Your parent must meet the financial limits and NFLOC criteria, but there's no separate CPS score requirement.

CFC + COPES (Home-Based)

If your parent can remain at home or in an Adult Family Home, these programs cover personal care, home health, adult day care, meals, and respite. Many families use this combination to delay or avoid facility placement entirely.

Adult Family Homes

Many AFHs (licensed for 2-6 residents) accept Medicaid. They don't participate in SDCP specifically, but Medicaid pays a daily rate based on the CARE assessment classification. For parents with moderate dementia, an AFH can provide a more personal care environment at a lower cost than an ALF.

The Financial Eligibility Hurdle

The numbers that matter for 2026:

  • Income limit: $2,982/month gross (300% of SSI). Exceed this? Washington uses a medically needy spend-down — no Miller Trust needed. Your parent spends medical/care expenses until remaining income hits $994/month, then Medicaid picks up the rest.
  • Asset limit: $2,000 countable (single applicant). Exempt: primary home (up to $1,130,000 equity), one vehicle, household goods, irrevocable burial contracts.
  • Spousal protection: Community spouse keeps up to $162,660 in assets and a minimum of $2,705/month income.

What Medicaid Doesn't Cover

Even with full Apple Health coverage, your parent still pays:

  • Room and board in an ALF: $885.26/month, paid from your parent's income
  • Personal needs allowance: $108.74/month retained for personal expenses
  • Non-covered services: Private rooms (if only semi-private available), companion services, premium amenities

Steps to Get Medicaid Memory Care Coverage

  1. Verify financial eligibility using the thresholds above — account for the spend-down option
  2. Apply for Apple Health through Washington Connection or a local DSHS Community Services Office
  3. Request a CARE assessment — this determines both functional eligibility and the CPS score needed for SDCP
  4. Prepare for the assessment — document all care tasks provided at least three times in the week before (the CARE tool only counts tasks performed 3+ times in 7 days)
  5. Ask the case manager specifically about SDCP and which facilities hold EARC-SDC contracts in your area
  6. Tour contracted facilities before placement — SDCP eligibility doesn't guarantee a bed at the facility you prefer
