Delaware Dual Eligible: Medicare and Medicaid Together

When someone qualifies for both Medicare and Medicaid in Delaware, they become what the system calls "dual eligible." But that term covers a wide range of coverage levels, and the difference between the categories matters enormously for what a beneficiary actually receives.

QMB-Only vs Full Dual

The distinction that trips up most families is between QMB-Only and Full Dual Eligible status.

QMB-Only means Delaware pays your Medicare cost-sharing — Part A and Part B premiums, deductibles, and coinsurance — but you do not receive full Medicaid benefits. You keep your Medicare coverage exactly as it is, and the state eliminates your out-of-pocket Medicare costs. That is it. No Medicaid dental, no Medicaid vision, no Medicaid long-term care, and no enrollment in a Medicaid managed care plan.

Full Dual Eligible (sometimes called QMB Plus or SLMB Plus) means you meet both the Medicare Savings Program income criteria and the categorical/financial requirements for full Delaware Medicaid. This gives you the complete Medicaid benefit package — adult dental, long-term care services, Home and Community-Based Services (HCBS) through DSHP-Plus, and managed care enrollment under the Diamond State Health Plan — on top of the Medicare cost-sharing protections.

The income and disability criteria for full Medicaid are different from those for the Medicare Savings Programs. Being approved for QMB does not automatically make you eligible for full Medicaid. They are separate determinations, though Delaware DSS evaluates both through the same ASSIST application.

How Coverage Layers Work

For Full Dual Eligibles, Medicare is the primary payer for hospital stays, doctor visits, and outpatient services. Medicaid wraps around as the secondary payer, covering costs that Medicare does not — long-term care, dental, certain home health services, and Medicare cost-sharing amounts.

The practical effect: a Full Dual Eligible person in Delaware has virtually zero out-of-pocket healthcare costs for covered services. Medicare covers acute and preventive care, Medicaid covers long-term and supplemental services, and the two together eliminate copays and deductibles.

For QMB-Only beneficiaries, the coverage is narrower but still powerful. Medicare handles all medical services. The state pays the premiums and cost-sharing. The beneficiary pays nothing for Medicare-covered services, but does not have access to Medicaid-specific benefits.

Extra Help and Prescription Coverage

Both QMB-Only and Full Dual Eligible beneficiaries automatically qualify for Full Extra Help under Medicare Part D. This means:

  • Part D plan premiums covered up to the regional benchmark amount
  • No Part D deductible
  • Copays capped at $5.10 for generics and $12.65 for brand-name drugs in 2026

Full Dual Eligibles may also have prescription coverage through their Medicaid managed care plan, which can cover drugs that fall outside their Part D formulary.

The Estate Recovery Question

This is the fear that keeps families up at night: will the state take the house after my parent dies?

QMB-Only, SLMB, and QI benefits are exempt from estate recovery under federal law. Delaware cannot recover any costs paid through the Medicare Savings Programs — no liens, no claims against the estate, nothing. This protection is explicit in 42 U.S.C. § 1396p(b)(1)(B)(ii).

Full Medicaid long-term care is different. If a beneficiary received full Medicaid-funded nursing facility care or HCBS waiver services at age 55 or older, Delaware can pursue estate recovery for those costs. The recovery applies only to the long-term care expenditures, not to any MSP payments made alongside them.

For families worried about this distinction, the key question is whether the parent receives long-term care services funded by Medicaid. If they are QMB-Only and living independently, estate recovery does not apply.

Getting the Right Determination

Delaware DSS evaluates both MSP and full Medicaid eligibility through a single application on the ASSIST portal. If your parent qualifies only for an MSP, DSS approves that tier. If they also meet the criteria for full Medicaid, DSS can approve both simultaneously.

The important thing is to apply. Even if full Medicaid seems unlikely, the MSP determination happens through the same process and can deliver substantial financial relief on its own.

Our Delaware Medicare Savings Programs, Extra Help & DPAP Guide walks through both the MSP and full Medicaid pathways, with an eligibility worksheet that helps you identify which programs your parent may qualify for before submitting the application.
