DC DHCF and Health Care Ombudsman Contact Information — Who Handles What

DC splits healthcare administration between agencies in ways that confuse even people who've been navigating the system for years. You call one office and they tell you it's the other office's problem. Knowing which office handles which issue saves hours of circular phone transfers.

Department of Health Care Finance (DHCF)

DHCF is the District's Medicaid agency — the policy and payment side. They don't process applications (that's ESA), but they control the rules, manage provider reimbursements, and handle the Part A/Part B buy-in payments that make QMB work.

Contact DHCF when:

  • Social Security is still deducting Part B premiums more than 60 days after QMB approval (DHCF Buy-In Coordinators can issue a manual buy-in transaction code)
  • You need to verify that QMB eligibility data was transmitted to CMS
  • You have questions about Medicaid policy — what's covered, what's not, how programs interact

DHCF contact:

  • Phone: (202) 442-5988
  • Address: 441 4th Street NW, Suite 900S, Washington, DC 20001

DC Office of the Health Care Ombudsman and Bill of Rights

The Ombudsman's office handles complaints about health care coverage, access, and bills. It works independently within DHCF and can help consumers navigate the relevant complaint or appeal process.

Contact the Ombudsman when:

  • A doctor or hospital bills your parent for Medicare deductibles, copays, or coinsurance despite QMB status (this is illegal under federal law — 42 U.S.C. § 1396a(n)(3)(B))
  • A provider refuses to accept the QMB designation or DC Medicaid card
  • A medical provider sends a bill to collections for amounts that should be covered by QMB
  • You need help filing a formal complaint against a healthcare provider or facility
  • You're having trouble getting a health plan or provider to process a claim correctly

Ombudsman contact:

  • Phone: 1-877-685-6391
  • The office handles complaints at no cost

DHS Economic Security Administration (ESA)

ESA is where applications actually get processed. For anything related to submitting, checking on, or fixing a QMB application, renewal, or case, ESA is the starting point.

Contact ESA when:

  • You need to apply for QMB or check application status
  • You received a renewal packet and need help completing it
  • Your parent's address changed and needs updating
  • You need to set up authorized representative access
  • You have questions about document requirements

ESA contact:

  • Public Benefits Call Center: (202) 727-5355
  • Fax: (202) 671-4400
  • Mail: Case Record Management Unit, P.O. Box 91560, Washington, DC 20090

ESA Service Centers (in-person, M–F 7:30 AM – 4:45 PM):

  • H Street: 645 H Street NE — (202) 698-4350
  • Taylor Street: 1207 Taylor Street NW — (202) 576-8000
  • Anacostia: 2100 Martin Luther King Jr. Ave SE — (202) 645-4614
  • Congress Heights: 4001 South Capitol Street SW — (202) 645-4525
  • Fort Davis: 3851 Alabama Ave SE — (202) 645-4500

Office of Administrative Hearings (OAH)

OAH is the independent tribunal that hears appeals. If ESA denies a QMB application or terminates benefits and you want to contest the decision, OAH is where the appeal goes — not back to ESA.

Contact OAH when:

  • You want to file a fair hearing request after a QMB denial or termination
  • You need to request "aid continuing" (benefits maintained during appeal) — this must be filed within 30 days of the notice
  • Standard fair hearing deadline: 90 days from the notice postmark date

OAH contact:

  • Email: [email protected]
  • Fax: (202) 724-2041
  • Phone (to initiate): (855) 532-5465
  • In person: 441 4th Street NW, Suite 450 North, Washington, DC 20001 — (202) 442-9094

The Right Order for Common Problems

"SSA is still deducting the premium after QMB approval" → Start with DHCF Buy-In Coordinators at (202) 442-5988. If the deduction continues more than 60 days after QMB approval, contact DC SHIP at (202) 727-8370 for advocacy.

"A doctor sent a bill for a copay" → Contact the provider's billing office first with the QMB approval letter. If they refuse to zero it out, escalate to the Ombudsman at 1-877-685-6391.

"QMB was denied or terminated" → File an appeal with OAH at [email protected] within 90 days (30 days if you want aid continuing). Get free legal help from Legal Counsel for the Elderly at (202) 434-2120.

The DC Medicare Savings Programs & Extra Help Guide includes the full contact directory, escalation workflows for each common problem, and template language for billing dispute letters and appeal requests.
