Big Sky Rx and Extra Help: The Filing Sequence That Unlocks Both

Why Big Sky Rx Applications Get Stuck

Over half of all pending Big Sky Rx applications sit in limbo for the same reason: the applicant didn't attach their Social Security Extra Help decision letter.

If your gross income falls at or below 150% of the federal poverty level — $1,816/month for an individual, $2,450/month for a couple in 2026 — Big Sky Rx requires proof that you've applied for Extra Help first. The Senior and Long Term Care Division needs either your approval letter or your denial letter from SSA before they'll process the Big Sky Rx application.

The state mails a notice explaining this requirement, but it often gets overlooked or confused with other correspondence. Meanwhile, the application just sits in pending status, and the premium help you need doesn't arrive.

The Correct Filing Order

Step 1: Apply for Extra Help through Social Security. Go to ssa.gov/medicare/part-d-extra-help, call 1-800-772-1213, or visit your local SSA office.

Step 2: Receive your SSA decision. You'll get a letter stating whether you qualify for Extra Help. Keep this letter — you need it regardless of the outcome.

Step 3: Submit your Big Sky Rx application with the SSA letter attached. Apply online at BigSkyRx.MT.gov or mail Form BSRx-002 to the Big Sky Rx Program in Helena. Staple or scan the Extra Help determination letter with your application.

Step 4: If the SSA letter is delayed, call Social Security at 1-800-772-1213 and request a copy of the determination. You can also ask your local SSA office to print a verification letter while you wait.

What If Extra Help Is Denied?

A denial doesn't kill your Big Sky Rx application — it actually clears the path. Big Sky Rx needs to know that Extra Help won't cover your Part D premium so it can step in with its own subsidy of up to $50.60/month. Submit the denial letter to the Senior and Long Term Care Division exactly the same way you'd submit an approval.

If Extra Help is approved, Big Sky Rx coordinates as a secondary payer. Extra Help covers your Part D premium up to the benchmark amount. If your chosen plan's premium exceeds the benchmark, Big Sky Rx pays the difference — up to its $50.60 cap.

What If Your Income Is Above 150% FPL?

If your gross monthly income is above $1,816 (individual) or $2,450 (couple), you don't need the Extra Help letter at all. Big Sky Rx accepts your application directly, as long as you're under its own income limits of $2,660/month individual or $3,606.66/month couple.

In this case, skip straight to the Big Sky Rx application. The Extra Help sequencing requirement only applies to lower-income applicants.

Unsticking a Pending Application

If your Big Sky Rx application has been sitting without a response, call the Big Sky Rx helpline at 1-866-369-1233 and ask whether they're waiting on an Extra Help determination. If they are, submit the letter by fax or mail and ask them to confirm receipt.

The Montana Medicare Savings Programs guide maps out the complete dual-filing sequence for all three programs — MSP, Extra Help, and Big Sky Rx — so nothing falls through the cracks.
