Arkansas QMB Processing Time: How Long It Takes and When Coverage Starts
How Long Arkansas DHS Takes to Process a QMB Application
Arkansas DHS has 45 calendar days to decide on a Medicare Savings Program application. That clock starts the day your county DHS office receives the application — whether you filed through the Access Arkansas portal, dropped it off in person, or mailed it in.
If DHS cannot verify information through its electronic data sources or finds a discrepancy, it may send a request for additional documents. Respond promptly to avoid delaying the decision.
To help avoid verification delays, submit complete bank statements (all pages, all accounts) and a current Social Security award letter with your initial application. Don't wait for DHS to ask.
When QMB Coverage Actually Starts
This is where many families get surprised. QMB has a strict effective-date rule that works differently from SLMB and QI.
QMB coverage begins on the first day of the month after DHS approves you. If DHS approves your application on March 15, coverage starts April 1. There is no retroactive QMB coverage — federal regulations prohibit it. Medical bills or premiums from before your effective date stay your responsibility.
SLMB and QI work differently. Both can be retroactive up to three months before your application date, as long as you met all eligibility requirements during those months. If you applied in April and qualified since January, SLMB or QI coverage can reach back to January.
This distinction matters if your parent is deciding between programs. Someone who clearly qualifies for QMB should apply immediately — every month of delay is a month of premiums and cost-sharing that won't be covered retroactively.
What Happens to the Part B Premium After Approval
Once DHS approves QMB (or SLMB or QI), the state sends an electronic accretion transaction to CMS. CMS then tells SSA to stop deducting the Part B premium from your parent's Social Security check.
This process takes 60 to 90 days to complete. During that gap, SSA keeps deducting the Part B premium even though coverage is approved. Don't panic — this is normal.
When SSA finally updates its records, two things happen at once: the monthly deduction stops, and SSA deposits a retroactive lump-sum reimbursement covering every premium withheld between the QMB effective date and the date the deduction actually stopped. You can verify this on the SSA monthly benefit statement, which itemizes the reimbursement as a separate line.
If it's been more than 90 days and the premium is still being deducted, call the DHS client assistance line at 1-855-372-1084 and ask them to confirm that the buy-in transaction was submitted. Sometimes the electronic file needs to be resent.

How to Track Your Application Status
The Access Arkansas portal at access.arkansas.gov shows your case status once you're logged in. Look for updates under your healthcare coverage application. If the status says "Pending Verification," DHS is waiting for documents — check for mail from your county office.
You can also call your local DHS county office directly. Have your application date and case number ready. If you designated an authorized representative, that person can call on your behalf.
Getting the Process Right From Day One
If DHS uses the full 45-day decision period, allow another 60 to 90 days for SSA to stop the deduction and issue the credit for premiums withheld since the coverage effective date. Filing a complete application can help avoid verification delays.
Our Arkansas Medicare Savings Programs & Extra Help Guide walks through the exact documents DHS needs, how to avoid the most common verification delays, and what to do at each stage while you wait for the premium to stop.
