Arkansas Benefits for Seniors: Programs That Help With Medicare, Prescriptions, and Living Costs
Medicare Cost Help: The Program Most Arkansas Seniors Miss
Arkansas Medicare Savings Programs (QMB, SLMB, QI) pay part or all of a senior's Medicare costs. QMB covers the Part B premium plus deductibles and copays. SLMB and QI cover just the Part B premium — still worth about $2,435 per year back in your parent's Social Security check.
Income limits are higher than many families expect. In 2026, an individual earning up to $1,816 per month (after a $20 disregard) can qualify for QI. A couple can earn up to $2,455. Most seniors on Social Security alone fall somewhere in the qualifying range.
Approval for any Medicare Savings Program automatically triggers Extra Help, the federal program that caps prescription drug copays at $4.90 for generics and $12.65 for brand-name drugs under Part D.
Applications go through the Access Arkansas portal at access.arkansas.gov, any county DHS office, or the DHS phone line at 1-855-372-1084.
Free Medicare Counseling Through SHIIP
The Senior Health Insurance Information Program (SHIIP) provides free, one-on-one Medicare counseling through trained volunteers across Arkansas. They help compare Part D plans, explain Medicare Advantage options, and review Medigap policies — all without selling insurance or earning commissions.
SHIIP counselors can help determine whether your parent qualifies for Medicare Savings Programs, but they can't complete the DHS application or represent anyone in an appeal. Reach them at 1-800-224-6330.
Prescription Drug Help Beyond Extra Help
Arkansas doesn't have a general state pharmacy assistance program (SPAP) for seniors. Extra Help through Medicare Savings Programs is the primary prescription cost relief available.
Beyond Extra Help, some options exist for specific situations:
- Medicare Part D out-of-pocket cap: All Part D enrollees have prescription costs capped at about $2,000 per year under the Inflation Reduction Act. Extra Help recipients reach even lower copay caps.
- Manufacturer Patient Assistance Programs: Most major drug companies offer free or low-cost medications for qualifying patients. Your parent's doctor or pharmacist can help identify these.
- Arkansas Kidney Disease Commission: Limited drug assistance for end-stage renal disease patients.

Home and Community-Based Care
For seniors who need help at home but want to avoid a nursing facility:
- ARChoices Waiver: Provides personal care, home-delivered meals, adult day health, and other services for Medicaid-eligible seniors who meet a nursing-facility level of care. This is full Medicaid, with stricter income and asset rules than Medicare Savings Programs.
- Independent Choices: A self-directed option under ARChoices that lets the senior or their family hire and manage their own caregivers.
- Meals on Wheels and senior centers: Local nutrition and social programs coordinated through the Area Agencies on Aging. Call the Arkansas Aging and Disability Resource Center at 1-866-245-5498.
Social Security and Medicare Together
Most Arkansas seniors rely on Social Security as their primary income. The monthly check determines both their living budget and their eligibility for cost-help programs. A few things to know:
The Part B premium comes straight from Social Security. At $202.90 per month in 2026, it takes a meaningful bite from a check that might be $1,200 to $1,600 before the deduction. Medicare Savings Programs eliminate this deduction entirely.
COLA increases don't automatically disqualify you. When Social Security goes up each January, a temporary protection (the COLA disregard) prevents benefit loss before the new poverty guidelines are published.
The Social Security Fairness Act raised some checks. For people affected by the old Windfall Elimination Provision or Government Pension Offset, the Act repealed those offsets for benefits payable from January 2024 onward; SSA began retroactive adjustments in February 2025. If your parent's check went up, they should confirm whether they still qualify for their current MSP tier — and if they're now over QMB limits, they may still fall under SLMB or QI.
Where to Start
For most Arkansas seniors on a fixed income, Medicare Savings Programs are the highest-impact benefit to apply for first. Part B premium relief follows each program's effective-date rules; SSA may take 60 to 90 days to stop the deduction, then credits premiums withheld after the effective date and before the deduction stops. Extra Help follows automatically, and the application doesn't affect your home or trigger estate recovery.
