The Expedited Appeal Timeline
Here's what happens once you call:
- You contact Acentra by midnight on the proposed discharge day. This is the deadline for a fast appeal. If you miss it, you can still ask Acentra to review the case, but different rules and time frames apply and you may be responsible for the hospital stay after the original discharge date.
- If you appeal on time, the discharge process stops immediately. The hospital cannot move your parent out, and Medicare continues covering the stay while Acentra reviews the case.
- The hospital must hand you a Detailed Notice of Discharge. This document lays out the specific clinical reasons the hospital believes your parent is ready to leave — lab values, vital signs, clinical assessments.
- Acentra reviews the full medical record and renders a decision within 24 hours of receiving all necessary records from the hospital.
If Acentra rules in your parent's favor, the discharge is canceled and Medicare coverage continues. If Acentra upholds the discharge, financial liability for the hospital stay shifts to your parent at noon on the calendar day after the decision.
What Triggers the Appeal Right: The Important Message from Medicare
Before any of this happens, the hospital is required to deliver a standardized federal notice called the Important Message from Medicare (IM). This notice must arrive within two days of admission and again within two days of the projected discharge. It lists your parent's rights and includes Acentra's contact information.

Appeals in Skilled Nursing Facilities and Home Health
The appeal process works differently in post-acute settings like SNFs and home health agencies. When these providers plan to end Medicare-covered services, they must issue a Notice of Medicare Non-Coverage (NOMNC) at least two calendar days before the last covered day.
To contest the termination, you must contact Acentra by noon on the day before the effective end date listed on the NOMNC. The provider then issues a Detailed Explanation of Non-Coverage, and Acentra renders its decision within two days.
A common trigger: the SNF claims your parent has "plateaued" in therapy. Federal policy prohibits denying coverage solely because a patient isn't improving — skilled care is covered when it's necessary to maintain current function or prevent deterioration.
If Acentra Denies the Appeal
You still have options. The next level is a reconsideration through Maximus Federal Services (the QIC for Part A West), which conducts a fresh review of the medical evidence. Beyond that, the Medicare appeals process includes an Administrative Law Judge hearing, the Medicare Appeals Council, and federal district court.
Most families don't need to go past Acentra. But knowing the escalation path exists gives you leverage when negotiating with the discharge planner.
