What Alternative Care Does

Alternative Care (AC) is a state-funded program — not federally funded — that provides home and community-based services to Minnesota seniors who meet the clinical need for nursing home care but are not yet financially eligible for Medical Assistance. It functions as a bridge: your parent receives waiver-like services while the family works toward Medicaid eligibility through a compliant asset spenddown.

The program covers many of the same services as the Elderly Waiver — homemaker assistance, personal care, adult day programs, personal emergency response systems, and caregiver respite. It does not cover room and board in an assisted living facility.

The 135-Day Eligibility Calculation

Alternative Care uses a specific formula to determine financial eligibility. Your parent qualifies if their combined countable income and assets are less than the projected cost of 135 days of nursing home care.

Using the 2026 Statewide Average Payment for Skilled Nursing Facility care (SAPSNF) rate of $11,869 per month:

AC threshold = $11,869 × (135 ÷ 30) = $53,410.50

If your parent's combined countable assets and income fall below approximately $53,411, they meet the 135-day financial threshold for Alternative Care — even though their assets may still be above the $3,000 Medical Assistance limit. The financial screen also includes a 120% Federal Poverty Guideline threshold, and the county must confirm the clinical and other program requirements.

This is the key difference: Medical Assistance requires assets at or below $3,000. Alternative Care catches families in the gap between $3,000 and $53,411, providing state-funded services during the months it takes to spend down to the Medicaid threshold.

Who Qualifies

To qualify for Alternative Care, your parent must:

• Be age 65 or older
• Meet Nursing Facility Level of Care (NFLOC) — determined through the MnCHOICES assessment
• Have combined countable income and assets below the 135-day threshold (~$53,411 in 2026)
• Not yet qualify for Medical Assistance for Long-Term Care

The MnCHOICES assessment is the clinical gateway. Without an assessment showing that your parent meets NFLOC, the financial eligibility calculation does not matter — the clinical requirement comes first.

How AC Coordinates with the Elderly Waiver

Alternative Care is designed as a transition program. Here is how the typical pathway works:

1. Your parent meets NFLOC through the MnCHOICES assessment
2. Countable assets are above $3,000 but below the 135-day threshold
3. The county enrolls your parent in Alternative Care
4. The family executes compliant spenddown expenditures — prepaid burial contracts, home modifications, outstanding medical bills, formal caregiver agreements
5. When countable assets reach $3,000, the county can reassess your parent for Medical Assistance and the Elderly Waiver

Do not assume the transition is automatic: submit the Medical Assistance application and coordinate with the county before funds run out. Families who prepare the application paperwork before reaching the $3,000 threshold experience smoother transitions than those who wait until the moment of eligibility.

Temporary AC for Pending Medicaid Applications

If your parent's liquid assets are already below $3,000 and a Medical Assistance application is pending, the family can request temporary Alternative Care for up to 60 days to prevent a gap in services while the county processes the application. This prevents the scenario where your parent qualifies on paper but waits weeks or months for the formal approval while receiving no funded services.

Contact your county human services department or call Minnesota Aging Pathways at 1-800-333-2433 to request a MnCHOICES assessment. The assessment establishes clinical eligibility; the county lead agency and financial worker then evaluate financial eligibility and route your parent to Alternative Care, the Elderly Waiver, or Essential Community Supports.
